. The following documents and information have been cited and complied with:

RD Conformed: Dr. Macabangjit P. Ati, DPA, Campus OIC
 Letter of application/intent dated November 4, 2019
A. Corporate and Administrative Documents  Special Order No. 03, S. 2016, dated December 21, 2016 Granting of Authority to the different departments
of MSU-MSAT to register or seek accreditation with TESDA to offer TVET Programs.
 Republic Act. No. 2408, An Act to convert the Maigo High School in the Mun. of Kalambugan, Province of
Lanao, into a Trade School, authorizing the appropriation of funds therefor.
 Original Certificate of Title No. 201700174 was shown as proof for land ownership.
 Fire Safety Certificate No. R10-0502188 valid until April 7, 2022
 Course Design and Modules of Instruction are compliant to the Training Regulation of Electrical Installation
and Maintenance NC III.
 Equipment, supplies and materials needed for the conduct of training are functional yet lacking.
B. Curricular Requirements  Progress Chart and Achievement Charts with unit of competencies were presented.
 Copies of Institutional assessment tools and results, and trainees record book presented.
 No Property Custodian/Tool Keeper was appointed to ensure the in and out of the tools, equipment and
materials through log book. Instructional materials, internet access and library resources essential in
delivering the program are maintained.

 Faculty, personnel and non-teaching staff were submitted to the Provincial Office with valid supporting
C. Faculty and Personnel documents presented and available. Also, periodic updates on the changes made were submitted to PO.

The current fees and grading system match with the registration documents submitted in the Provincial Office. Entry
D. Program Guidelines requirements specified in the application documents compliance with the TR are on file and are required upon
enrolment. Schedule of fees, proof of grades and rules on attendance is provided to each trainee during orientation.

 With permanent school nurse, Amerah Mahira M. Ati, and School Dentist, Dr. Norania U. Bucay who
facilitated and assisted students with their medical needs.
E. Support Services  Provision of Career Guidance Services through: Counselling, Training Induction Program, Power Test, and
Job Induction Program were presented.

Program Performance Measures Training is going on during the conduct of the virtual compliance audit.

 Provisions of Health and Safety protocols against covid-19 crisis was strictly observed and implemented.
 Tools and equipment/materials are lacking during the conduct of the virtual compliance audit.
Other Observations  No designation of tool keeper to ensure the in and out of tools and its completeness.
 No transmittal letter stamped received by the PO for the changes and updates for the list of new officials,
trainers and non-teaching personnel.
 No submission of digitized copy of the student’s credentials to the Provincial Office after completion of the
training program with transmittal stamped received by PO focal.
