Subject: Request for Reimbursement of Antigen Swab Test

I would like to inform you that I am John Clark P. Gregorio, Teacher-I from ESP department of
Lawang Bato National High School, writing this letter to request you for reimbursement of the
medical swab antigen test that was spent by me on March 15, 2022 at Medical Healthcare
Clinic, April 19, 2022, and May 16, 2022 at Blue Edge Medical Biomedical Services for the
requirement of on-site reporting in school amounting the sum of two thousand two hundred
eighty pesos (₱ 2,280). Therefore, I request you to kindly reimburse the mentioned amount.
