Dear Madam,

I am Sarah Jane L. Mantos, 30 years of age, a teacher from Logon National High
School, Logon, Daanbantayan, Cebu for four years now.

I am grateful with the employment that is given to me; however I feel the need to be
closer to where my family resides. In connection with this, I would like to express my
intent to be transferred to Tapilon National High School, Tapilon, Daanbantayan, Cebu.

These are the reasons for my humble request.

1. I am a resident of Tabunok, Tapilon, Daanbantayan, Cebu.

2. The school where I am presently assigned is quite far from my residence.
3. Travelling expenses in going back and forth to Logon NHS really affects my financial
status.
4. As a mother of a 6 year-old child, I would like to fulfill my responsibility,
and attend to his utmost concerns.
5. I want to serve your institution more and share my knowledge and skills and I feel it is
best shared with my fellow Tapilohanons.

My pertinent papers are attached herewith. I look forward for your prompt and favorable
response on this matter. I am willing and able to come for an interview anytime
