Good day!

I am Ana Mae Edullantes a student nurse from davao doctor’s college and today I’ll be
showing to you my return demonstration.

So first is I will performed hand hygiene to deter the spread of microorganism and I will applied gloves to
my hands.

I will greet the client politely and I will introduce myself and will ask the client on how the client would
like to be called during the assessment to establish rapport.

So now I will close the door to ensure the client’s privacy.

Inquire if client has any existing and history of cardiovascular problem, as well as lifestyle habits that are
risk factors of cardiac problems.

So for assessing for Jugular Veins.

So now I am the right side of the client with the client at a supine position with the head of the bed
elevated between 30 and 45 degrees, making sure that the head and torso are on the same plane. And
Instructed the client to turn the head slightly to the left and shone tangential light source onto the neck,
suprasternal notch and area around the clavicles to observe for pulsations and shadows.

For assessing for Carotid Arteries, with head of the bed still slightly elevated at 30 degrees, positioned
the client's head slightly towards the side being examined. Palpated the carotid artery cautiously
avoiding to much pressure or massaging the area. Repeated the preceeding step on the other side.

Now for Inspection and Palpation of the heart, Began with general insection of the chest wall.

Simultaneously inspected the precordium for pulsations while palpating aortic area, pulmonic area,
tricuspid area and apical area. So Palpated for heaves and lifts using the palm and/or hold fingerpads flat
or obliquely against the chest. Inspected and palpated the epigastric area at the base of the sternum for
abdominal aortic pulsations.

Now for auscultation Assessed for heart rate and rhythm by placing the diaphragm of the stethoscope at
the apex and listening closely to the rate and rhythm of the apical impulse. Counted the heart beat for a
full minute. And Using the diaphragm of the stethoscope first, then the bell, auscultated the heart in all
four anatomic sites: aortic, pulmonic, tricuspid, and apical (mitral) for heart sounds, extra heart sounds,
and murmurs. Asked the client to breathe regularly while auscultating.

Now I will examine for the Upper and lower extremities of the client. Assessed each arm for size,
symmetry, skin color and temperature from fingertips to shoulder. Noted for any presence of edema,
lesion, changes in skin texture and hair distribution and I will palpat for radial pulse, ulnar pulse, and
brachial pulse individually and bilaterally. And I will Assessed for capillary refill then I will also Performed
the Allen Test.

Then perform again hand hygiene.
