    I, the undersigned parent/guardian allow my son/daughter, EDELSON WINDEL C.


    RAPADA with ID Number 19100613, from the College of Commerce, Secretarial &
    Accountancy to undertake the different activities necessitated by the requirements of
    his/her enrolment in ACCOUNTANCY INTERNSHIP, during the First Semester, SY
    2022-2023, which includes among others the following:

    • Processing and/or Submission of OJT/Internship Requirements


    • Consultation with Advisers
    • Maintain confidentiality of information obtained

    I expect that the College through the personnel-in-charge shall exercise due diligence
    which includes oral or written instructions required for the safety and well-being of my
    child throughout the duration of the activity.

    In the event that my child disregards or fails to follow instructions or should act on
    his/her own, or the happening of any unforeseen event, I together with my child, shall
    have no claim against the College, should any damage be caused, or liability be incurred
    to property or person.
