Dear Parents & Guardian,

We would like to inform that Koolkidz Academy has included an insurance fee in the
school package for enrolment 2022 onwards. This insurance is for students from age
2 – 6 years old.

This insurance coverage is called School Personal Accident (SPA) under Zurich
Insurance and the cost is RM30.00 yearly per students.

It will cover any accidents during / in school premises. The accidents will cover based
on the insurance plan. Attached to this memo is the Form and benefit plan for parents
to sign.
