Dear GWENDALYN DELOS REYES BIBAS :

This pertains to your application to the 2021 DOST-SEI Undergraduate Scholarship Program.
 
Due to the emergence of the new COVID-19 variants and in consideration of the health, safety, and
security of both the applicants and test personnel, holding the scholarship examination is
impossible. As such, the DOST-SEI shall consider your grades as one of the indicators in the
selection and identification of scholarship qualifiers.
 
In this regard, we will communicate with your school to request for your grades. Should you allow
DOST-SEI to process your personal information to be provided by your school, kindly accomplish
the attached Consent Form (FOUND BELOW) and upload it in the E-Scholarship Application System
using this link on or before 31 March 2021. You can download, re-type, take a photo, use a
digitized signature, print the document, etc.; just make sure that you upload the signed Consent
Form in the system so we can process your application.
