The offices formulate measures & provide technical assistance to the Local Chief Executive in
the insurance of delivery of basic services. Its operational components includes child development &
protection; nutrition service; youth welfare & development; especially welfare program for elderly &
disabled; social upliftment services; disaster relief assistance & inter-agency coordination on social
welfare & development for local government offices & non –government organization.
FRONTLINE SERVICES
Mondays - Fridays from 8:00 AM - 12:00 noon & 1:00 PM - 5:00 PM
The Social Case Study Report (SCSR) is given to client in order t avail of assistance from the
Philippine Charity Sweepstakes Office (PCSO): other government hospitals, institution & non-
government organization, Mapandan Community Hospital: Region1 Medical Center, Dagupan City;
National Children’s Hospital ; St. Luke Medical Center (Medical Social Service Department) ;
Pangasinan Prov’l. Hospital , San Carlos City Pangasinan; PGH;BGH; PNMH; Philippine Heart Center;
Kidney Center; Lung Center; Trauma Center; DSWD Heaven ; DWSD- AVRC1; DSWD FO1; Regional
Rehab. Center for Youth(RRCY); GMA Kapuso foundation ;& Others.
Service Schedule:
Monday to Friday 8:00 AM to 12: 00 Noon; 1:00 PM to 5: 00 PM
FEES: None
Requirements:
• Certificate of Indigency from the Office of the Barangay Captain.
• Medical Certificate; Clinical Abstract; Medico-Legal if CICL or Case & Police Report or
Police Blotter.
• Birth Certificate
• Whole body picture
• Psychiatric Eval. Report (When needed)
