ASSESSMENT GUIDE
Please refer to the information on the assessment guide and portfolios that has been provided
online before commencing this activity.
This checklist addresses Generalist nursing practice. Where a procedure is not applicable to your
area, tick N/A.
1. This checklist must be used in conjunction with your professional portfolio (Portfolium)
development. It underpins your clinical learning during this unit of study. Please check the
instructions provided for this activity on Moodle.
2. As part of your learning plan, you need to identify the competencies and skills that you
need in your specialty area, then rate yourself on each of them. Remember, a low score
merely means that you need more work on your skills and knowledge, so please be honest
with yourself. Use this assessment to plan your learning for the term.
3. You need to complete this assessment twice during this term.
a. The first time is in Week 2 to determine where you are in regard to your clinical and
professional skills development.
b. The second time is to reflect with your nurse educator/mentor on what you have
learned during the term (to be completed in Week 9).
4. Submit both self-assessments in Moodle, in the tab provided.
5. After it has been reviewed ensure both the Week 2 and Week 9 self-assessments have
been added to your Portfolium by Week 11.
NOTE:
If the procedure or skill is not applicable to your area, tick not applicable (N/A).
You may also add locally required activities to this list.
