1.0 INTRODUCTION
Essentially, fire is a chemical reaction. A carbon-based material (fuel) mixes with oxygen (usually a
component of air) and comes in contact with something hot enough to heat this mixture so that combustible
vapors are produced. If these vapors dissipate, then nothing happens. However, if they come in contact with an
ignition source--such as open flame--a fire results. Depending on the combustibility of the ignited fuel, the fire
may start as a slow-growth scenario with a long smoldering period or it may grow rapidly with almost no
smoldering time. In either instance, once visible flames appear, the fire's destructive forces increase exponentially.
We as a group have unanimously voted for the tragedy of the fire that occurred a petrochemical plant
in Pasir Gudang caught firewhich occurred on 17 February 2022 around 5.16 p.m.

2.0 CAUSE OF FIRE
The cause of the fire that took place in this plant was due to the material involved in
the fire being cyclohexane and a mixture of mertodaine contained in Tank A, Tank B and
Tank C. Tank A and Tank B were on fire. Cyclohexan (also known as CYX, hexamethylene,
hexahydrobenzene, hexanaphthene, and benzenehexahydride) is a colourless, volatile, and
flammable liquid with the formula C6H12. It has a mild odour and is insoluble in water but
soluble in alcohol, ether, acetone, benzene, and ligroin. Cyclohexane occurs naturally in
petroleum crude oil, volcanic gases and cigarette smoke. Industrial cyclohexane can be
produced by two methods. The first is the catalytic hydrogenation of benzene using rhodium
on carbon, and the second method is via fractional distillation of petroleum. Cyclohexane has
a specific gravity of 0.78 and a flashpoint of -20° C and is highly flammable. It should be
stored in a cool, dry, and well-ventilated area which is free from the risk of ignition. For
transportation purposes, it is classified as hazard class 3 and packing group II and is should
be labelled as an irritant, and harmful if swallowed or inhaled. Cyclohexane is used
predominately in the nylon industry where approximately 90% of it is consumed in the
industrial production of adipic acid and caprolactam, which are themselves used to generate
nylon6 and nylon6.6. The remaining 10% is used both as a solvent for paints, resins, varnish
and oil, and as a plasticiser.

4.0 FIRE SAFETY PLANNING
1. Make sure the fire alarm is working and can be heard clearly.
3. Keep the emergency exits open and make it easier for employees to escape.
4. Ensure that the employer registers all employees to obtain insurance to make it easier
for the employee in the event of an accident.
5. Make sure not to smoke or burn in the factory area due to flammability.
