    A drilling rig experienced an accident where the derrick was pulled over. The sign instructing the driller to hang it on the brake was in the doghouse but not hung. The rig manager was on location but not on the floor overseeing operations. The new driller was on the brake but attempted to come out of the hole with the rams still closed, pulling the derrick over. Pictures were attached of Rig 149's fallen derrick as a safety lesson.

    A LITTLE PLANNING GOES A LONG WAY!!!

    IF . .  they had just hung the sign on the BRAKE!!!!

    "Or . . .   How to Junk a rig in less then 10 seconds!!!"

    Subject: NOT HAVING A GOOD DAY

    The sign was in the doghouse but not hung…….the rig manager was on
    location but not on the floor …….the driller was new and was on the brake.

    The rams were still closed when the driller attempted to come out of the
    hole.

    Attached are the pics many of you have requested of Rig 149's derrick
    pulled over.

    Good info to use as a safety meeting topic or general talking points in your
    travels and discussions with rig managers and crews.
