Filipinos, and residents of Iligan City, after having duly sworn to an oath as required by
law, hereby depose and say;

1] That we personally know the person of CHERRY ALNGOHURO LARA;

2] That we also know the fact that either the names be, CHERRY ALNGOHURO
LARA or CHERRY LARA a contributing member of Pag-ibig Fund under the employer’s
name: INDRA GARMENTS MFG., refer to one and the same person of CHERRY
ALNGOHURO LARA MAQUIDDANG as married person;

3] That we are executing this affidavit to attest to the above facts and for such
other legal purpose it may serve best.
