BOARDING HOUSE AGREEMENT

KNOWN ALL MEN BY THESE PRESENT:

This Agreement is made and executed this __________ day of _________ in the City of Tacloban, by and
between:

STREETLIGHT PHILIPPINES INC. a juridical entity integrated at the Security and Exchange
Commission with office address at Corner Senator Enage Street and Magsaysay Boulevard, Tacloban City
represented by _________________________________ of legal age, Filipino, and residing at
_____________________________________________ known as the First Party.

And

____________________________________, owner of the Boarding House known as the Second Party.

WITNESSETH

WHEREAS, The First Party is the guardian of

____________________________________________.

WHEREAS, The Second Party is the owner of a boarding house situated at

____________________________________________________________.

WHEREAS, The First Party offered to rent a room from the Second Party for the above
mentioned name (s), and the Second Party agreed to the same with the following conditions:

(1.) Rent: Advance Payment of ________________________________

One month deposit of _______________________________
Due date every ____________________________________
(2.) That the Second Party will strictly implement a curfew hours until 10pm in the evening to
the occupant.
(3.) That in the case the occupant will violate the curfew hours, then the Second Party is obliged
to report to the First Party the violated incident for proper actions.
