General Instruction: The items below consist of identified community health problems with associated
diagnosis. Each identified problems is treated separately according to the set of criteria, to determine the
appropriate ranking for each diagnosis. Complete the table by following the steps given:

1. Inaccessible Water Supply


 57 households main source of water is from jet water pump
 2 households use water from unprotected dug well and 2 from shallow dug well
 Refilling station are about 2.7 km away from barangay.
