1. Why is the family considered as the basic unit of the society? (5 points)
Because family is the first who fulfill our basic needs, such as shelter, food and clothes, it is from
the that we start making social relationships, develop and retain such relations.
2. What are the unique features and characteristics of a family that influence in community and
community health? (5 points)
Based on the Philippine Constitution, Family Code with focus on religious, legal, and cultural
aspects of the definition of family.
3. As a community health nurse, describe your involvement in family health nursing in the
community. (5points)
It involves a set of action by which the nurse measures the status of the family as a client. Its
ability to maintain wellness , prevent, control or resolve problems in order to achieve health and
wellness among its members.
4. What are the ways by which community health nurses get into contact with the family, and
how are theydone? (5 points)
Community health nurses work in small towns, cities or rural areas. In addition, they are
employed in a variety of healthcare settings. You may find community health nurses in places
such as clinics, churches, community health centers etc.

Let’s test how much you understood the concepts in this module through this practice case study.

1. Watch the i-Witness documentary, “Manaram,” by Kara David.

2. Choose one pregnant woman interviewed by Kara David as your “virtual client,” and perform a simple

First Level and Second Level Assessment.

3. Follow the format below in accomplishing your assessment to your client (based on the data elicited
by

Kara David, the BHW or the Manaram).

4. Using the data that you have “gathered”( based on the documentary), identify at least three Family

Nursing Diagnoses.

5. Suggested reference: Maglaya, A. S. (2005). Nursing practice in the community (4th ed.). Argonauta

Corporation. Pages 74-81.

Situation:

(Narrate your client’s health situation, using First Level Assessment. Take note of the health history,

chief complaint, laboratory tests, and your own assessment of the client based on what you have
observed

while watching the documentary. Get as much detail as possible).

Situation:

From Barangay Gusa, It takes four to five hours of walking to reach the town’s nearest rural health clinic.
And because of the trek downhill is long and perilous, most pregnant women choose to give birth with
the help of the “manaram” . However the the government ask the Community health nurse/midwife to
travel on barangay Gusa and arrange a medical mission for them. The midwife was going over the record
files of the patient, and checked the file with the prenatal appointment book. A 17 year old patient,
Name Angelica, on her first pregnancy, for Angelica she prefers to give birth at the hands of doctors and
nurse.

Findings:

Blood Pressure: 90/80

High risk for bleeding

Pale skin

Weak and lethargic

She doesn’t know her last menstrual period (LMP)

Management: She need ultrasound and laboratories

The available data gathered during the first-level assessment reflected the following possible health

condition or problems (give at least three):

If given the chance to conduct an in-depth second-level assessment with your chosen client, what

questions would you ask her to support the possible health condition or problems that you have
identified above? Give at least five questions.

4. Do you take any vitamins?

5. Do you have any exposure to infectious diseases?

1. Discuss the importance of observing the Bag Technique for the Community Health Nurse.
Incorporate the principles surrounding the Bag Technique. (10 points)

The importance of observing the bag technique for the community health nurse is to be able to
perform a nursing procedure with ease and deftness, as well as to save time and effort while providing
nursing care to clients, and principles of avoiding infection transfer are always observed, we have to
avoid having contaminated tools by doing a wrong procedure, performing the bag technique will
minimize, if not prevent, the spread of any infection.

2. Explain the purpose of Home Visit, and how it aids the Community Health Nursing in planning,

implementing, and evaluating the family nursing care plan. Incorporate the principles surrounding the

Home Visit. (10 points)

A nursing home visit is a contact between a family and a nurse that allows the health worker to assess
the home and family situation in order to provide the necessary nursing care and health-related
activities. It is critical to prepare a visit plan in order to meet the needs of the client and achieve the
best results of desired outcomes when performing home visits. The purpose of home visit is to assess
the living condition of the patient and his family and their health practices in order to provide the
appropriate health teaching. And also to establish close relationship between the health provider and
the public for the promotion of health. And last and most importantly to give health teaching
regarding the prevention and control of diseases. And the principle of home visit, A home visit must
have a goal or purpose.

When planning a home visit, use all available information about the patient and his family from family
records.

When planning a home visit, we should consider and prioritize the individual's and his family's basic
needs.

Individuals and families should be involved in the planning and delivery of care.

The strategy should be adaptable.
