MEMORANDUM OF AGREEMENT

MEDICAL MISSION

KNOW ALL MEN BY THESE PRESENT

This agreement entered into by and between:

ST FRANCIS GENERIC DRUG –PASO DE BLAS, a duly organized and existing under
the laws of the Republic of the Philippines with office address at Stall 13-14,
Sentro Market, Paso De Blas, Valenzuela City, represented herein by its Owner, Lhemmuel
M. Fiesta, hereinafter referred to as the “CLIENT”.

AND

STO. DOMINGO DIAGNOSTIC AND MEDICAL CENTER CORP., a corporation duly

organized and existing under the laws of the Republic of the Philippines with office/clinic address
at Room 303-305 Luz Building, No. 111 Quezon Avenue, Barangay Sto. Domingo, Quezon
City, represented herein by its President, JOEL E. PEREZ, hereinafter referred to as the
“CLINIC”.

WITNESSETH:

WHEREAS THE CLIENT desires to engage the services of the CLINIC to render the Medical
Mission.

WHEREAS, the CLINIC provides medical and diagnostic services with qualified physical staff,
adequate facilities and services required by the CLIENT to conduct the Medical Mission.

NOW THEREFORE, for and in consideration of the foregoing, the parties have agreed the
following terms and conditions:

1. That schedule of Medical Mission shall be conducted on December 5, 2021 from 5:00 am up to
10:00 am.

2. Medical Mission Package Cost shall be TWO HUNDRED EIGHTY PESOS (PHP280.00) per
patient, which includes the following:

a. CBC
b. Urinalysis
c. ECG
d. Blood Chemistry(FBS, TOTAL CHOLESTEROL, BUN, SGPT and URIC ACID)
e. Free Platelet Count

3. That Package cost shall be paid in cash.

4. That Minimum guaranteed patients per schedule shall be at least fifty (50).

5. That Medical Mission Procedures shall be performed by CLINIC and data results shall be kept with
utmost confidentiality.

6. That X-RAY procedure is optional . Mobile X-ray unit shall be stationed at the COMPANY’s
designated area. Electricity shall be provided by the CLIENT.

7. That Results shall be delivered by CLINIC to the CLIENT after one week from the date
of Medical Mission, with free doctor’s consultation.
IN WITNESS WHEREOF, the parties hereto have affixed their signatures in Quezon City this 23rd day
of February 2022.
