for hypertension and dialysis that you filed before the 8888 Citizen’s Complaint Hotline (Ticket
Reference Number P20220315-387-9-3).

Please know that the undersigned is more than willing to provide assistance for your medical
expenses. You and/or your authorized representative are always free to visit the Office of the
City Mayor anytime between 8 AM to 5 PM so that we can discuss matters regarding your
request. You just need to bring documentary requirements such as valid ID, Certificate of
Indigency, Medical Certificate/Abstract, and medical prescription.

In case there is anything else that the undersigned and the City Government can help you with,
please do not hesitate to tell us.
