Following are some management failures which could be the contributing factors in this accident:
1. Outdated safe system of work and risk assessment
It is mentioned in the scenario that these documents were produced 5 years ago. Most work-related accidents
occur by following poorly produced safe system of work.

2. Inefficient audit of management system

As form the scenario, manager is responsible for internal audit of management processes. His inefficient audit
might not have helped him in recognizing the faults of management system like poor resource allocation.

3. Work Pressure
As clear from scenario, because of too much work workers are pressurized to complete work as quickly as
possible. This can result in errors in work, which could lead to accidents.

4. Vacant positions for too long

There is an increase in sickness rate mentioned in the scenario because of overwork as there are a lot of vacant
positions so employee workload increases and can result in accidents.

5. Ignoring of workplace concerns of workers

The management ignored the concerns and protests of workers as seen in scenario. This can lead to low
employee morale and can cause accidents.

6. Not allocating enough resources

During investigation in scenario the driver mentions that the job required more manpower and they asked
office but they no one was available to help. One factor of accident is the less allocation of resources.

7. Insufficient job consultation with workers.

The workers involved in accident believe that management do not consult properly with them about job. This
lack of consultation develops lack of trust and miscommunication. This could be a reason of accident.

8. Not learning from past experiences

In scenario, there was an accident in past followed by a civil claim. The management should have learned from
that experience and strengthened its health and safety system.

9. Non-priority of health and safety of workers

they employ less people to cover more jobs and earn more profit. It puts workers in danger. And could result in

10. Non-investigation of past accident

There was an accident in past and only notice was issued. There was no investigation to address the root cause
of the accident. It could be another factor leading to accident.
