ACCESS TO LGU ENERGY CODE/EVOSS ONLINE FORM

I. Create a new Google Mail (Gmail) account following the format below:
<region>. evosssurvey@gmail.com
Example: region1.evosssurvey@gmail.com, car.evosssurvey@gmail.com

This gmail account will be used for LGU Energy Code/EVOSS Monitoring.

II. Send an e-mail at evoss.dilg@gmail.com using your new Gmail account with
subject <Region> <New Google Account>. Or, if already available, send it
to us with the given format.

III. The EVOSS Focal Person will give the region access to the online form.

IV. Only registered Gmail account can modify/encode information to their

regional form:

1) Has the LGU issued the Ordinance Adopting the Guidelines to Facilitate
the Implementation of Energy Projects?
➢ Encode 1 if Yes or 0 if No
➢ If yes, provide an electronic copy (scanned copy in pdf format)
(City/Municipality to submit the electronic copy thru email to
C/MLGOO, then C/MLGOO submit to DILG Provincial Office and
