CONTRACT OF SERVICE

Known all men by these presents:

This agreement made and entered May 19-21, 2016 in Calumpit, Bulacan by and
between

DepEd POCMES, School of Calumpit North District an Educational Institution and

existing under law, with office address at Sta. Lucia, Calumpit Bulacan, represented by
AURORA T. CALING, OIC, Filipino, at legal age, married and resident of Sto. Niño,
Calumpit Bulacan herein called

FIRST PARTY,

and

MR. LARRY MAXIM, of legal age, married and resident of Malolos, Bulacan,
herein after the Second Party.

That the First Party proposes to the Second Party for the transportation on GENDER
AND DEVELOPMENT at Vigan, Ilocos Sur on May 19-21. That the payment in the said
services, the Second Party will collect the amount of Nine Thousand Nine Hundred Eighty
and fourty four cents (Php. 9,984.40) to cover up the transportation for the GAD Seminar

In witness whereof, the parties affirmed their signature this May 19-21, 2016 at
DepEd Calumpit, Bulacan.
