Address Details – Please fill in the complete residential address as per the supporting
document that you will submit. Also, mention the nearest landmark like school, temple,
petrol pump etc. to locate the same.
Academic Details – Please fill in the complete details of the education you wish to declare
as per your supporting documents. Please mention the Roll/Enroll/Seat No., passing year
etc. as per the details mentioned on the documents.
Employment Details – Please fill in the complete details of the companies that you have
worked with. Name and contact details of the company HR is not mandatory however
address of the company office where you have worked is required.
Reference Details – Please provide the details of the person who will be connected for
your reference check. Contact number is mandatory for the same.
Letter of Authorization – Please download the LOA, print and manually sign the same to
upload it again along with your supporting documents. Manually signed LOA is mandatory to
be uploaded else your submission will be rejected.
Supporting documents – Please upload the scan copy of all your supporting documents
