AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, GERTRUDES SAMSON FUENTES, Filipino, of legal age, married and a resident of
Sambog, Corella, Bohol, and HELEN Q. IGOT, Filipino, of legal age, married and a resident of
Tagbilaran City, Bohol, after being sworn in accordance with law, hereby depose and say:

That we are the friends and thus personally know the person of JULIET CADENAS
SIPSIP;

That JULIET CADENAS SIPSIP was born to Spouses Lorenzo Cadenas and Matilde
Caderao on May 18, 1972 at Tabuan, Garcia-Hernandez, Bohol;

That the fact of birth of JULIET CADENAS SIPSIP was recorded in the Register of Births
of the Office of the Civil Municipal Registrar of Garcia-Hernandez, Bohol;

That by virtue of these presents, we hereby attest that JULIET CADENAS SIPSIP was
born at Tabuan, Garcia-Hernandez, Bohol on May 18, 1972 to Spouses Lorenzo Cadenas and
Matilde Caderao;

That we are not related to the said person either by consanguinity or affinity and that we
have no interest in whatever outcome these presents may bring unto her;

That we execute this Affidavit voluntarily to attest to the truth of the foregoing statements
for whatever legal purpose this may serve.
