1. Printed Self Learning Modules (SLM) or Learning Activity Sheets (LAS) are provided by the school.

Get SLM/LAS
and return AS in the school every Friday of the week at 8:00 A.M. to 4:00 P.M. (Strictly only parents, guardian
or any adult are allowed to come to school).
2. Please do not write anything on your SLM or Learning Activity Sheets (LAS). Write your answers on the Answer
Sheets attached. However, if Answer Sheets are not provided, just answer immediately on the LAS.
3. Return only the accomplished AS or LAS and leave the SLM at home but to be collected at the end of the
quarter as part of your portfolio.
4. The school advocates honesty and diligence in answering the SLM/LAS hence, all subject areas require a
portfolio from each student which will be included as part of the Performance Task. Your Portfolio shall be
graded according to: Completeness of SLM and AS -10 points; Punctuality of Submissions- 10 points; Neatness
of Work – 10 points; and Honesty in Answering SLM- 20 points; a total of 50 points.
5. In checking your AS or LAS. Keep track of the following symbols which will be written in your AS if observed by
the subject teacher:
CC – COPY PASTE (same answer with another student) NYP – NOT YOUR PENMANSHIP
6. You may consult or ask questions concerning your lessons or class at your class Group Chat or directly to your
teachers whose contact information are written above.
