I, the undersigned, would like to request to transfer. Current station is Lower Mainit
Elementary School, am hereby writing this letter to allow me to transfer to your school
Ditucalan Elementary School, I would like to transfer because of the following
grounds:

1. I have a severe nearsightedness, and it is dangerous for me to drive motorcycle by

myself going to school.

2. I have a sinusitis that trigger me every day when I woke up inside my classroom
because of the weather we have there in Lower Mainit, the coldness I felt every morning
that trigger my sinusitis and the dust in the road that I encountered when going to
school.

3. I believe that I will be an effective teacher if I will have less worries and burdens
especially on my health issues.

4. Ditucalan Elementary School is near my residency and this is my Alma Mater

School.

I would really appreciate if you consider my request and grant me this permission, am
always ready for any process that I might be required to undergo for this to be
complete.
