We, ________________________________________, the undersigned and the parents
of _____________________________________of grade ____hereby give our written consent
that we both as a parents are ready to send our ward to school from __________________at
our own risk & responsibility.

We are fully aware about the COVID-19 RISK FACTORS. The School management has
briefed us on the necessary safety; precautionary and preventive measures the School is
planning to take and we are fully convinced and satisfied with it.

We are very well aware of the current COVID-19 Pandemic situation and the
uncertainties related to it. The differences in opinion of medical fraternity on exact medical
treatment to be given in case of individual infected person. This in some cases can lead to
physical, psychological health issues (in spite of best possible preventive measures taken and
medical treatment available) which in rare cases can even result into mortality. We hereby
confirm that under any circumstances we will not hold the SVYEMS and its Management
responsible for infection, illness or any unfortunate mishap resulting into mortality. We are
sending the ward to school solely on our responsibility.

We further state that we will fully support the School policies and safety measures taken
by school and will advise and instruct our ward to follow it.

We are aware about the online classes’ option but we voluntarily want to send our ward for
the offline classes.

This consent is given by me voluntarily and without any physical and psychological
pressure from anyone.
