CERTIFICATION
In connection with the appointment dated August 22, 2020 of OMAPAS,FRANK
TIONGSON as Teacher II (Senior High School) of the Department of Education-Divison of
Sultan Kudarat, Sangay National High School, this is to certify that:

a. Efforts to recruit, screen and select appropriate candidates have been fully
exerted;
b. Qualified LET passers fron the division’s Registry of Qualified Applicants (RQA)
per DepEd Order No. 3, s. 2016 have been exhausted; and that the appointee(s)
is/are qualified per DO3, s. 2016;
c. Upon deliberation of the qualified appliacants, no candidate matched the
requeirements of the position to be filled in the division; and
d. He is qualified to the said position and exceeded the basic requirements having
obtained a VERY SATISFACTORY performance in the last rating period and
having obtained a National Certificate II in Cookery, National Certificate II in
Commercial Cooking and a Trainer’s Methodology Certificate I holder. He has
competent skills in his field, his manisfestation of strong desire to be of good
service to the school and the community despite of the distance of residence of
the school.

Issued this 22nd day of August 2020 as an additional requirement to support the
