Dear FS students,

The following will be the requirements before you will be deployed for Field Studies in your
chosen laboratory school:

1. Photocopy of your vaccination card/Latest RTPCR Test if not vaccinated yet (to be repeated
upon requirement of the Laboratory School)
2. Photocopy of the receipt of your insurance that will cover COVID sickness
3. Photocopy of loadslip this semester
4. Medical Certicicate from a government doctor indicating that you are physically fit to
conduct Field Studies in DepEd Elementary/Secondary Schools)
5. Parent/Guardian’s Consent and Waiver (I will give sample -to follow)

Please submit these documents in a long folder with an accent of the color pasted front of the
folder (any design of pasting will do) assigned to your major. THis is for easy sorting. Submit
the folder at the faculty room in the specific date assigned to your class to avoid crowding.

Please be guided accordingly.

Note: You are asked to prepare more or less P 500 each of you since you are to give honorarium
of 300 to your mentor, and the reamining will be gathered together for the honorarium of the
Superintendents, Division Student Teaching Coordinators and Principal of the schools involved.
This will be gathered/given by December 2022 already. You just prepare in advance and keep it
in your savings.
