CERTIFICATION

This to Certify that Jevie M. Carzon, of legal age, Filipino, is a resident of Kalipayan St.
Poblacion District 3, La Paz, Leyte.

It is upon his request that we issue this Certification that last December 16, 2021
(Thursday), Typhoon Odette (RAI) hit the municipality and brought intense winds and rains and
caused all residents to leave their houses and evacuate. The Municipality and the whole
province of Leyte is under the State of Calamity under Proclamation No. 1267 issued by the
President of the Republic of the Philippines.

This Certification has been issued for all legal purposes or intents this may serve .
