ACTIVITY TITLE: PORTFOLIO DAY (Fourth Quarter)
IDENTIFYING INFORMATION
Date: April 11, 2018
Venue: Bangon Elementary School
Participants’ Description: All Teachers, Pupils and Parents
ACTIVITY DESCRIPTION
Objective: To conduct parent-teacher adviser conference for feed-backing of
pupil’s performance.
HIGHLIGHTS
To celebrate the said topic for the following reasons:
1. As per Regional Memorandum No. 327, s. 2017, entitled “Observance of the
Synchronized Regional Portfolio Day” to be done after every grading period.
further provides the conduct of the School-Based Portfolio Day shall be done
every quarter with the following suggested activities:
Distribution of cards;
Parents-Teacher-Adviser conference and Feedback-giving;
Exhibit/display of pupils/students outputs and performances; and
School PTA Meeting
The following were the activities done in the said day:
It started with a prayer through multi-media presentation;
Singing of the National Anthem;
Reciting of Panunumpa sa Watawat;
Reciting of DepEd Vision, Mission and Core Values;
Attendance Recognition and Message by the school head; and
Portfolio Proper was followed.
RESULTS
Achievement of Objectives: The objectives were attained.

Participants’ Feedback: The parents and pupils were very happy about the said
activity. The parents was informed that portfolio day is an important day to
showcase the accomplishment of the pupil’s best outputs in school.
1. Design of the Event
Parents and pupils were very thankful for the efforts made by the teachers.
2. Methodology
Distribution and signing of pupil’s report cards;
Display of pupils/student outputs;
Conduct PTA Meeting; and
Parent-Teacher-Adviser Conference and Feedback-giving.
3. Management of the Activity
The event was hosted by our teachers of Bangon Elem. School. Without the
presence of our school head because she is attending the seminar we
implement the said activity. It was a meaningful day to our teachers and
stakeholders.

Everybody was very attentive and participative in showing their interest to
know the importance of portfolio day.

No financial contributions was involved.

After the opening ceremony, the parents started roaming


around to the different grade levels booth were his/her child
enrolled in, to see the outputs/performance of their child for the
Fourth Quarter. They signed the report card of their child for the
Fourth Grading Period and they are happy to see their outputs.
