(HPTA) MEETING HELD AT GRADE 1 – GOLD CLASSROOM, ESPERANZA
CENTRAL SCHOOL, ESPERANZA, SULTAN KUDARAT ON ____________.

Present:
Parents/guardians of Grade I-Gold class (please see attached attendance sheet)

After the quorum has been ascertained, the meeting was started at 8:30 in the
morning with an opening prayer and the singing of the National Anthem. The class
adviser Mrs. ______ warmly welcome the parents/guardians and delivered her opening
remarks and statement of the purpose. She then presented and thoroughly discussed the
following agenda:

After the presentation and discussion of the meeting agenda, the class adviser led
the election for the new set of HPTA officers. And since the new set of officers was
already organized, the class adviser turn-over the floor to the newly elected HPTA
President to preside the second part of the meeting.

During the conduct of the meeting, the HPTA officers and members unanimously
agreed and approved the following projects, activities and agreements:
