AFFIDAVIT
WE, SPOUSES RUTH MILLADO and RIC MILLADO, both of legal age, married, Filipino, and
residents of Brgy. Cudionan, San Dionisio, Iloilo, Philippines, after having been sworn to in
accordance with law, hereby depose and say that:

1. We are the co-borrowers/co-maker of the late ERNESTO B. CANONG, under the Chattel
Mortgage at BDO, with Promissory Note No. 501130202062, who is the registered owner of a
motor vehicle known as Ford Fiesta 4DR MID, which is more particularly described as follows:

2. That LOURDES CANONG is the lawful and legal wife and the sole surviving heir of the
late ERNESTO CANONG;

3. That in connection with her intention to transfer the registration of the aforementioned
motor vehicle in her name, WE, hereby voluntarily and willingly give our consent to said transfer;

4. That WE are executing this Affidavit to attest to the truthfulness of the foregoing facts
and to support LOURDES CANONG’s application for the transfer of registration of the above-
described motor vehicle;
