QUITCLAIM and WAIVER

I, Ruth Evaristo, Filipino, of legal age, married/single, with

residence at Unit 3948, SMDC Blue Residences, Katipunan Ave.,

Quezon City, hereby executes this Quitclaim and Waiver under the

following conditions:

1. That the sum I paid to Hayen Interiors Design Services

(Hayen Interiors) will be refunded to me in full, in the total

amount of Thirty Four Thousand Two Hundred Pesos (P

34,200.00), receipt of which is hereby acknowledged to my

complete and full satisfaction.

2. That I can use the documents provided by Hayen Interiors

for Evaristo Residence 3948 Project for whatever purpose, to

wit:

a. Schematic Design Perspectives;

b. Interior Design Working Drawings;

c. Scope of Work; and

d. Schedule of Interior Finishes.

3. That I hereby release and discharge Hayen Interiors and its

directors, officers, employees, and agents from any claim or

 otherwise may be due in connection with the Evaristo

Residence 3948 Project.

4. That I hereby state further that I have no more claims or right

of action of whatsoever nature whether past, present or

contingent against Hayen Interiors and its officer, directors or

employees and forever release Hayen Interiors from any and

all liability.

5. I have read and fully understood this Quitclaim and Waiver

which is given and made freely, willingly, voluntarily, and

with full knowledge of rights under the law without any

threat, violence, duress, intimidation, or under influence

exerted upon my person.
