FIRST LEVEL ASSESSMENT


The available data gathered during this first level assessment reflected the following
possible health condition or problems:
1. High blood
Supporting cues- 50 years old mother of 2 with BP of 140/100.
2. Presence of Breeding/ Resting Sights of Vectors of Disease
Supporting cues- Possible nesting ground of mosquitos, rats and other pests due to the
dump of garbage and drums near kitchen area.
3. Cigarrete Smoking
Supporting cues- Mother states there is a chance that her 14 year old son is smoking
discreetly.
4. Poor Home/Environment Condition/ Sanitation
Supporting cues - Inadequate living space
- Lack of facilities/ appliances
- Polluted water supply
- Presence of breeding or resting sights of vectors of diseases.
- Improper garbage disposal
- Poor lighting and ventilation
- Air pollution (outdoor kitchen)
5. Failure to Utilize Community Resources for Health Care due to:
-Lack of trust/confidence in the agency/personnel
6. Inability to recognize the presence of condition or problem due to:
- Lack of or Inadequate knowledge
7. Presence of stress points/foreseeable crisis situations
