Instructions:

Use the Risk Matrix on page 20 in the textbook to determine risk assessment codes (RAC) while completing the following:

1. Construct a System/Subystem Hazard analysis worksheet with a minimum of five (5) hazards. Use the format shown in Chapter 7 of textbook.
2. Complete a risk assessment for two (2) of the identified hazards perform a risk assessment including the following information using the same format at Chapter 7
examples.

Hazard Condition Item:

Item 1: Acceleration - Aircraft generators weigh more than 100 pounds and are awkward to maneuver during removal and installation. If
dropped they can crush the bones of an employees foot. The generators instability during the same process can cause it to shift;
pinching body parts against surrounding components.

Item 5: Loss of Cooling Oil - Oil is used to maintain acceptable temperatures inside of a generator during operation. A significant loss
or reduction in volume of this oil will impact the ability of the system to regulate temperature, causing the generator to fall off-line and
reducing the effectiveness of the electrical system.

Risk Assessment (refer to Tables 2.1 - 2.3 in Chapter 2):

Item 1: Acceleration - I have given this item a RAC of 2C based on the potential for moderate to severe injury and the frequency of this
type of maintenance occurring; assuming there are no controls in place addressing this issue.

Item 5: Loss of Cooling Oil - I have given this item a RAC of 2D based on the server impact a lost generator would have on the
electrical system and unlikelihood of the oil lines failing.

Detailed Recommendation to reduce risk:

Item 1: Acceleration - The use of a jack or cradle would allow the removal and installation of the generator without employees
physically handling the weight of the generator.

Item 5: Loss of Cooling Oil - Clamps are placed on hydraulic lines in order to prevent flexing and chaffing. Inspections should be
conducted in order to ensure these clamps are in the correct place and in good enough condition to do their job.
