Cavite do hereby APPOINT Elvira S. Ezpeleta as Agent /Attorney-In-Fact) widow , likewise of
legal age, with postal address at Blk. 18 Lot 2 Villa Remedios Subdn, Salitran 2, Dasmarinas, Cavite
as my true and legal representative to act for and in my name and stead perform the following acts:

To secure Certificate of Registration (COR), Authority to Print (ATP) for Sales

Invoice of my Mini Restaurant at my business / residential address with Bureau Of
Internal Revenue, RDO 54A Trece Martirez, Cavite.

HEREBY GRANTING unto my representative full power and authority to execute and

perform every act necessary to render effective the power to secure COR and ATP for my
Mini Restaurant., as though , I have so performed it, and HEREBY APPROVING ALL that
she may do by virtue hereof with full right of substitution of her person and revocation of
this instrument.
