COVID-19 WAIVER

KNOW ALL MEN BY THESE PRESENTS:

As the parent or legal guardian of __________________________________, I hereby

acknowledge that I have been informed of the details of the conduct of school's year end
rehearsal on June 1 and Hybrid Ceremony on June 3, 2022, at 3 p.m.
I am voluntarily assuming any and all risks that notwithstanding the school's best efforts to
implement and require compliance with prevention and mitigating measures of the National and
Local Government, my child/ren, or any member of my family, may be exposed to the
coronavirus and may become ill with COVID-19 (or any of its mutations), and such exposure
and illness may result in personal injury, temporary or permanent disability, or even death. By
voluntarily assuming all the risks that attach to on-campus or in person education, I am holding
__________________________________ free and harmless of any unfortunate consequence
that a COVID-19 (or any of its mutations) infection may bring should my child/ren, or any
member of my family, contract the same while my child/ren is attending the said events while
the COVID-19 virus or any of its mutations is still lurking in the environment.
With full understanding, I — on behalf of myself, my household members, and my child/ren —
hereby freely and voluntarily give my consent to my children's participation in these activities. I
also attest that I had sought the views of my child/ren and he/she has expressed willingness to
participate in the activities.
