JOINT AFFIDAVIT OF TWO (2) DISINTERESTED PERSONS

WE, ROWENA REYES CRUZ and RODELIO LLANTINO CRUZ, both of legal
ages, Filipino, and both residing at Blk 4 Lot 31 Ph3 Summerwind Vill, Salitran III, Dasmarinas
City Cavite after having been duly sworn to in accordance with law hereby deposes and say:

That we personally know the late, APRONIANO DONDONILLIA who died on

October 3, 2003;

That late FE AIDA DONDONILLA TICA, who died on February 18, 2021 at
Dasmarinas City, Cavite is one of his children;

That based on our personal knowledge and his documents presented his name is
APRONIANO DONDONILLIA and also based on his late daughter Certificate of
Birth;

That however, in her late daughter’s Certificate of Death his name appears as
“PROPONIO DONDONILLIA” which there is a discrepancy in his first name;

That his correct and true name is APRONIANO DONDONILLIA based on his late
daughter Certificate of Birth and our personal knowledge;

That we hereby attest and testify that LU APRONIANO DONDONILLIA and

PROPONIO DONDONILLIA, as described herein, refer to one and the same person;

This affidavit is executed for the purpose of attesting our personal knowledge and for all
legal intents and purposes;

IN WITNESS WHEREOF, we have hereunto set our hands this 18 th day of July 2022 at
Dasmarinas City, Cavite.
