CONFIDENTIALITY & CONSENT FORM

INTERVIEW

Miss Ma. Kristina B. dela Cruz is interviewing respondents to learn more about the impact of gamification
and online laboratory courses on students' motivation, self-regulation, and physics performance.

Your responses in this interview will provide critical information that will aid in the instructor's teaching
style improvement. The researcher will ask questions and record the conversation during the interview.

All of the information you provide during your session will be kept completely private. The researcher may
store the notes for future use after the research is completed, but any personally identifying information you
submit for yourself or others will be removed.

Your participation in this research is entirely voluntary. You are allowed to ignore any questions or prompts,
as well as to end the session at any time. It will have no effect on your Physics grade whether or not you
opt to participate.
