CONFIRMATION OF WAIVER
That We, EDNA R. MORILLA and DANILO V. MORILLA both of legal
age, Filipino, a resident of Sitio Talaba, Brgy. Burgos San jacinto, Masbate,
Philippines, after having duly sworn in accordance with law, do hereby depose and
state that:
1. That we are the biological parent of ARJAY R. MORILLA;

2. That we are expressing our consent as regards our son’s plan of crossing
country (from Taiwan to Poland);

3. That this is incidental to his plan of looking for better employment

opportunity;

4. That we, with all good fidelity, willingly consented and agreed to his
decision, and to boost this claim, attached herewith is our hand written
consent executed before the notary public before Atty. Mary Lizza D.
Bartolata;

5. That we are aware of the possible risks and opportunities that he may
encounter of executing the same; and

6. Finally, that we are executing this affidavit attest to the truthfulness of

the foregoing facts/statements and for any lawful and legal purpose/s this
may serve.
