Province of Masbate, after having sworn to in accordance with law, do hereby
depose and state:

1. That, I am the sole proprietor of a retail store which I operate under the Business

2. That due to my advanced age, I can no longer manage the aforementioned store
and as such, I am now transferring the ownership and management of my store
to my son, ___________;

3. That the transfer of ownership and management of the store of my son is not to
defraud nor escape any liability, if any;

4. That, in view of the foregoing, I am executing this AFFIDAVIT to attest to the

truth of the foregoing and all other matters necessary and incidental for the
purpose for which this has been executed.
