3. SCOPE AND PURPOSE

The purpose of the Safety Program is to identify and correct the conditions
responsible for accidents and to avoid the resultant human suffering and economic
waste.

4. APPLICABILITY

5. RESPONSIBILITY

5.1. The General Manager will exert necessary efforts in commitment to the
Policy Statement.
5.2. Every employee of the company is required to play his part in promoting
accident prevention commensurate with his area of responsibility.
5.3. The Project Manager will familiarize himself with site safety regulations and
comply with them at all time:
5.3.1. The prime responsibility for site safety rests with the Project
Manager.
5.3.2. He should be fully conversant with clients, the country of
operation
Workman’s Regulations etc…
5.3.3. Ensure that all supervisors are also aware of and comply with
the safety regulations and accident prevention requirements.
5.3.4. Institute a system of inspection and correction by Supervisors of
potential hazards.
5.3.5. Hold weekly supervisors meetings at which safety problems will
be discussed.
5.3.6. Coordinate investigation and reporting of accident.
5.3.7. Discuss accident prevention recommendations made in site
Safety Engineer and initiate action.
5.3.8. Ensure an adequate supply of the correct types of personal
protective equipment is available.
5.3.9. Check continuity of safety training and weekly « Tool box talks

5.3.10. For example, promote and encourage safety awareness in
employees at all levels.

5.4. The site Safety Engineer is responsible to the Project Manager for the day to day
implementation of the Safety Program.
5.4.1. Carry out regular site safety audits and report and correct unsafe work
practices and potential hazards.
5.4.2. Investigate and report all accidents including near miss incidents and
recommend preventative measures.
5.4.3. Maintain records of all injuries and damage on site and submit reports
as required.
5.4.4. Prepare & submit monthly report
5.4.5. Assist with training.
5.4.6. Issue weekly « Safety Line » for display on bulletin boards.
5.4.7. Attend weekly supervisors meeting and discuss accident prevention
techniques and safety performance.
5.4.8. Ensure maintenance of good hygiene standards and availability of first
aid treatment.
5.4.9. Advise field supervision on safe working methods.
5.4.10. Enforce the observance of the Work Permit procedure.
5.4.11. Promote an understanding among the work force that accident
prevention is an indispensable part of any efficient industrial activity.

5.5. A great deal of the effectiveness of the company’s accident prevention effort will
depend on the attitude and approach of the Safety Officer.
5.5.1. Maintain good house keeping.
5.5.2. Inspect equipment and tools before use and ensure correct storage
facilities are available and properly utilized.
5.5.3. Enforce the use of applicable personal protective equipment.
5.5.4. Take action for unsafe conditions immediately when they arise.
5.5.5. Conduct induction training and weekly safety meetings with work
crews.
5.5.6. Attend weekly supervisors meetings.
5.5.7. Cooperate with site Safety Engineer to create and maintain safe
working conditions in all operations being performed.

5.6. The workman also has responsibilities for accident prevention, for not even the
best of supervisors can watch over each man every minute of the work day.

6. GENERAL

6.1. The basic tenet of the Safety Policy is that all employees have the
inalienable right to perform their duties in a safe and healthy working
environment, free from all recognizable hazards.

6.2. The implementation of the Safety Program can not be delegated as a


function solely of the site Safety Engineer. Every field supervisor is
accountable for the introduction and enforcement of safe working practices
within his area of responsibility.

6.3. A detailed list of regulations and corresponding actions against violations of


safety rules by any employees is to be developed distributed and applied
respect to site needs.

6.4. A monthly safety meeting is to be directed by the Project Manager and


attended by all staff. The agenda is to include an evaluation of safety
awareness and progress, a forecast for the risks of the next month activity
and check for sufficient control measures to be taken.

6.5. Access to site is to be available only through security checkpoints in order to


control and register the in and out traffic and protect the site against
intruders. These checkpoints are to keep record for all vehicle entries and
personnel as well as designation for visitors and other parties. All employees
are issued an identification badge.

6.6. The safety manager is empowered to “STOP WORK”, and to transfer this
empowerment to his safety assistant with respect to activities being
performed on site. The Project Manager shall instruct the execution team to
stop any activity when suspecting serious and immanent danger and to
inform safety team immediately.

6.7. Upon receiving a safety non-conformance report, logged in and distributed,


the Safety Manager shall immediately and within reasonable time to allocate
responsibilities to rectify and to verify sustainable actions were taken. After
closing the violation on site, the in-writing closure shall be replied through
document controller and logged out.

6.8. General site safety checklist is to be provided respect to site execution

6.9. Internal safety Audit is to be carried out by the Safety Manager on site and
reported to the General Manager every 4 months or at the need of reviewing
safety plan and implementation after an accident or introducing new
technology. A rating checklist is prepared for that purpose.

7. PROCEDURE

7.1. Incident Reports And Investigation

7.1.1. We are often able to identify future hazard areas and


apply preventative measures through analysis of
previous accidents. However, effective remedial action
can only be applied when the basic problem is clearly
defined. Investigation, submission of detailed factual
reports and the evaluation of these facts are the basic
tools for establishing the real cause of any accident and
directing our Accidental prevention activity in the most
efficient way.

7.1.2. Reports

7.1.2.1. An immediate oral report will be made to the


company site/area office and the Client’s
representative in the following cases :

All fatal injuries.


All injuries requiring hospital treatment.
All damage to plant or equipment in excess of
500 USD.
All damage to Client’s property in any amount
All fires.
All Cases of contagious or industrial related
diseases.
All near miss cases.

7.1.2.2. A full detailed written report of these incidents


must be submitted within 24 hours with copy to
client’s site representative. All other accidents
are to be recorded and monthly injury and
damage reports submitted with copies to Client
Loss Prevention in accordance with
Client’s procedures.

7.1.2.3. In addition any injury which incapacitates a man


for three days must be reported in writing for
submission in compliance with the labor law.

7.1.3.1. The initial responsibility for reporting an accident


rests with the supervisor of the operation where
the incident occurs.

Obtain treatment for any injured person.


Ensure that as far as possible the scene of the incident remains
undisturbed.
Report to the site Safety Engineer.
Describe the operation that was taking place and the sequence
of events, which led to the incident.
Assist the site Safety Engineer to examine any plant or
equipment involved in the accident and to
question witnesses.

7.1.4. Investigation

7.1.4.1. The responsibility to investigate all downgrading incidents,


submit reports and recommend corrective action is incumbent
on the site Safety Engineer.

Visit the scene of the incident and examine any plant or


equipment for damage or misuse.
Question witnesses as to that happened, how it happened and
why.
Take photographs or make sketches.
Question any injured person as soon as possible.
Ascertain the events that led to the incident, are any
irregularities in the operating.
Analyze the basic cause, report and recommend preventative
action.

7.1.4.2. Accident investigative techniques are many and all may be


successfully employed in varying circumstances the overriding
factor however must always be that accident investigations are
to determine the cause and preventative requirements NOT to
apportion blame.
7.1.4.3. Formats required by the client representative will be used and
submitted.

7.2.1. No accident prevention program can be considered complete


without the conclusion of a comprehensive training element.

7.2.2. The issuing and enforcing of safety rules and regulations will not
instill in the workman the desired attitude towards accident
prevention, this can only be created and fostered through
education.

7.2.2.1. All employees are given a short period of on-site


induction training with an integrated safety content.
This is normally given by the Safety Officer and will
include an identification of the common site hazards, as
well as those more specific to the particular craft and
indicate preventative measures to be taken to avoid
accidents.

7.2.2.2. All Safety Officers shall prepare a weekly safety talk


that they will give to the crews under their control.
These talks should each deal with a specific hazard
and should be mainly but not limited to unsafe work
practices as applied to the particular craft. Each talk
should be approximately ten minutes duration and
conclude with a short period for discussion and
suggestions.

7.2.2.3. The induction training program shall include:

Accessing Site facilities.


Inform employees for their Safety Rights & Duties
Area of Danger
Do & Do Not
Passenger Routes & Vehicle routes
Emergency Response
Required PPE for Various activities
Muster Points.

7.2.3. A good supervisor will be aware of most hazards as they arise


within his specific craft responsibility. However, even the best of
supervisors may become so familiar with a particular operation
under his contract that he may fail to notice a fall in standards
and developing hazard conditions.

7.2.3.1. A monthly safety meeting of all supervisors is to be


held by the Project Manager at which he will require the
site Safety Engineer to report safety performances and

areas. Supervisors shall be encouraged to participate
freely in discussions to improve site safety.

7.2.3.2. Bulletin boards are to be provided for the display of


posters and weekly Safety Line from the Safety
Engineer, these will be sited in areas where workmen
are likely to spend some time or pass frequently.

7.2.3.3. Selected personnel are to be required to undertake


specialist training in fire fighting, first aid and the use of
self contained breathing apparatus.

7.2.3.4. Supervisors should take an active part in this training to


improve their own performance and to demonstrate to
the work force the importance and to demonstrate to
the work force the importance attached to these
activities.

7.3. Welfare And First Aid

7.3.1. First aid facilities are provided at site and are supervised by the
attendant qualified male nurse who shall be employed
exclusively on First Aid and Medical duties.

7.3.1.1. He shall maintain adequate supplies of first aid


materials at all times.
7.3.1.2. Familiarize himself with local hospital availability and
telephone numbers.
7.3.1.3. Ensure that the site ambulance is maintained in sound
and clean condition.
7.3.1.4. Assist the company doctor during his regular visit to the
site.
7.3.1.5. Maintain a site register listing all treatments given.
7.3.1.6. When requested by the site Safety Engineer, give first
aid training to supervisors and crew leader.
7.3.1.7. Supply first aid kits to the leaders of any crews working
at a distance from the facility.
7.3.1.8. Report immediately to the Project Manager and site
Safety Engineer any cases suspected contagious
disease.

7.3.2.1. The provision of welfare facilities and the establishment


and maintenance of good hygiene at site is of major
importance. Good hygiene and good housekeeping go
hand in hand.

7.3.2.2. Trash of any kind if allowed to remain on site


constitutes not only a fire but also a hygiene
hazard. Mosquitoes, flies, rodents and snakes with
their attendant dangers to health will breed and
flourish in these conditions.

Trashcans are to be provided at locations through-


out the work site and must emptied
regularly.
Toilet and washing facilities with adequate water
supply are to be provided and must be
cleaned daily.
Sufficient water, and in hot weather, ice is to be
made available on site. Disposable
cups should be also provided.
A rest-area and, when necessary, smoking areas
are to be designated.
An adequate supply of hand cleaner and barrier
creams is to be maintained.
7.3.2.3. A third party, approved by local authority, is to be
hired to manage collection of waste and disposal in
respect to the environmental regulations.

7.4. Fire Prevention

In order to support combustion three things are required: fuel, heat and
oxygen; if we can exclude any one of these elements, fire cannot exist.
The easiest component to remove is fuel and this can be done by good
housekeeping and the regular and early removal from site of all
flammable scrap and waste materials.

7.4.1. Smoking shall be strictly prohibited in the execution area or


in any area other than designated that should be away from
flammable store.
7.4.2. No open fires are allowed on site.
7.4.3. Hot Work Permit is to precede any hot work on site.

7.4.4. Flammable Liquids

The flammable liquids present on construction sites are almost

fuel, oils, paint and solvents.

7.4.4.1. Flammable liquids must be stored either in secure


open compounds or well ventilated huts.
7.4.4.2. The various categories of liquid should be stored
separately and clearly marked.
7.4.4.3. Storage areas for flammable liquids are to be sited at
least 30 ft. from any building or other storage area and
50 ft. from the boundary fence or any Client facility.
7.4.4.4. No smoking or naked flames are permitted within 20 ft.
of flammable storage areas.
7.4.4.5. Flammable liquids must be stored in securely capped
or sealed metal containers.
7.4.4.6. Transporting of flammable liquids is to be done in
closed containers
7.4.4.7. The engines of all gasoline or diesel powered plant and
equipment are to be switched off during refueling.
7.4.4.8. Any spillage of flammable liquid must be contained with
sand and removed immediately off during refueling.
7.4.4.9. Flammable store shall be furnished with fire detector &
sprinkler.

7.4.5. Fire Extinguishers

7.4.5.1. The usual fire extinguisher issued to site is the 20 lb.


multi purpose dry powder, stored pressure or cartridge
type. These are effective on « A » « B » and « C »
fires.

7.4.5.2. It is not possible to state the number of extinguisher


required or their probable sitting as this is dependent
upon many factors and should be considered during
the planning stage and in consultation with site Safety
Engineer

7.4.6. Fire Fighting

7.4.6.1. A fire extinguisher is only as efficient as the person


operating it; therefore personnel are to be instructed in
their use and fire and evacuation drills carried out.

7.5 Personal Protective Equipment

7.5.1. K2 will provide all employees with items of personal


protective equipment appropriate to the task being
performed.

7.5.2. The use of such equipment when issued is mandatory on all


employees. Any employee who enters the site shall wear
helmet, safety shoes & goggles. Additional mandatory PPE
shall be used upon the result of Job Hazard Analysis and its
control measures.

7.5.3. Helmets

7.5.3.1. Safety helmets shall be issued to all employees and


must be worn at all times on the job site.
7.5.3.2. Helmets shall not be painted or drilled for any
purpose.

7.5.3.3. Any helmet displaying cracks, splits or severe


abrasions must be exchanged.

7.5.4. Gloves

7.5.4.1. Gloves of the appropriate type shall be issued to all


employees and should be worn whenever danger
exists of injury to the workers hands.
7.5.5. Goggles And Face Masks

7.5.5.1. Tinted and plain goggles shall be available for issue


to employees engaged in cutting, chipping and
grinding operations.
7.5.5.2. Welders’ helmets shall be issued to all welders.
7.5.5.3. Face shields shall be issued where full face
protection is considered necessary.

7.5.6. Foot Wear

7.5.6.1. The wearing of sandals, flip-flops or athletic


bumpers on site is prohibited. Only safety shoes are
allowed.

7.5.7. Full Body Harness

7.5.7.1. Full Body Harness shall be issued by all personnel

level where it is impracticable to erect a proper
scaffold or

Belts should be adjusted to fit snugly and in use must be fixed to a firm anchor
point. After use belts should be stored in a clean dry place. Supervisors will
inspect belts for incipient faults before each shift.

7.5.8. Ear Muffs

7.5.8.1. Ear muffs are supplied to operators of all noisy


equipment, jack hammers or rock drills and those
persons assigned as attendants of compressors
and generators.

7.5.8.2. Ear plugs are to be issued to personnel working in


conditions where the noise level may be considered
an aggravation.

7.5.9. Respirators

7.5.9.1. There are many types of respiratory protective


equipment each with a particular application. It will
be necessary to determine the individual
requirements of any operation which involves the
creation of fumes or dust or where a hazardous
atmosphere is likely to exist.

The type of operations and its probable duration.


Type of contamination, particulate, vapor or asphyxiate.
The location of the hazardous area.

7.6. Transportation

All drivers are expected to drive in a safe and defensive manner with
courtesy and consideration for other road users.

7.6.1. Requirements And Regulations

7.6.1.1. It is law that no person shall drive a motor vehicle


before obtaining a driver’s license applicable to that
type of vehicle and that such license must be in his
possession whilst driving that vehicle.

01 Drivers shall be issued with a copy of the


AREA TRAFFIC REGULATIONS and abide by
them.
02 No vehicle is to be driven in excess of the legal
speed limit and in accordance with the country
regulations.
03 All Government and site traffic signs are to be
complied with.
04 Vehicles are to be parked in designated
parking areas where provided.

7.6.2. Vehicles

7.6.2.1. It is the responsibility of the driver that his vehicle is safe to


operate and is properly maintained, by delivering it to
workshop for servicing at the correct intervals.

7.6.2.2. Drivers shall inspect their vehicles before use.

Steering and brakes satisfactory.


Tires correctly inflated and free from defects, including the
spare.
Seat belts installed and operable.
Horn working.
All lights including turn indicators operating.
Rear view mirrors correctly positioned.
Windshield wipers and washers correct and windshield
clean.
Check oil and water level.
Jack, wheel brace and warning triangle all available.
Fire extinguisher charged.

7.6.3. Passengers

7.6.3.1. Passengers may be carried only in the passenger


compartments of vehicles and only as many persons as the

Passengers must not mount or alight from moving


vehicles.

7.6.4. Accidents

7.6.4.1. A driver involved in a motor vehicle accident shall


not leave the scene or move his vehicle until
permitted to do so by the police except to remove
any injured persons to hospital, following which he
shall report to the nearest police station and if so
instructed return to the scene of the accident.

7.6.4.2. All traffic accidents, however minor, must be


reported to the Safety Engineer.

7.7. Plant And Equipment

7.7.1. K2‘s mechanical equipment will only be operated by


personnel trained in its use and who are in possession of a
valid license for that class of machinery.

7.7.2. It is responsibility of the plant supervisor to ensure that only


qualified personnel are allowed to operate equipment and
that all plant and equipment is maintained in a safe and
serviceable condition in accordance with Procedures.

7.7.2.1. Machinery should be inspected before being placed


in service and daily by the supervisor of the
operation employing the equipment.
7.7.2.2. The operator shall check oil, water, fuel and

7.7.2.3. All moving machine parts must be guarded.
7.7.2.4. Operators shall ensure that machinery is switched
off before any repair, adjustment or refueling is
commenced.
7.7.2.5. Drivers of mobile machinery must switch off the
engineand remove the ignition key before leaving the
controls.
7.7.2.6. Compressors, generators and other static
machinery must be sited in such a position that
exhaust fumes will not affect those working nearby.
7.7.2.7. Care must be taken to differentiate between the
requirements of air supplied to tools and that for
breathing.
7.7.2.8. Compressors must be operated strictly in
accordance with the manufacturer’s instructions
and when supplying air for breathing apparatus
must not be left unattended.
7.7.2.9. Generators should be inspected by a qualified
electrician to ensure that electrical connections are
properly made and the machine is grounded.
7.7.2.10. Before commencing work, the operator must check
the area around his machine for men or
obstructions.
7.7.2.11. Excavator operators must ensure a clearance of at
least 2 ft. from any object when excavating.
7.7.2.12. ALL reversing Heavy Equipment shall be attended
by a banks man.

7.8. Excavations

7.8.1. All excavations of a depth of 4 ft. or more require being


either shored or sloped back to prevent collapse of the
sides, the design of shoring or angle of slope shall be
determined by the supervisor of the excavating operation
and must comply with the requirements of the Safety
Manual.

7.8.2. Other factors affecting the required safety measures must


also be considered.

7.8.2.1. Where vehicles routes are established near

excavation area, a compacted soil barrier of


minimum 60 cm height shall be placed as a rigid
barrier, steel bars will be hammered vertically in soil
to stand along the routes connected by rope and
tagged by warning tapes, flash lights shall be
installed, and warning signs “DEEP EXCAVATION”
shall be distributed along the routes.
7.8.2.2. Where underground cables, pipes etc. are
suspected such obstructions must first be exposed
by hand digging before mechanical excavating
commences.
7.8.2.3. Mechanical excavators shall not be used within 10
ft. of any underground obstruction.
7.8.2.4. Men shall not be permitted to work where they may
be struck by mechanical excavator
7.8.2.5. Properly constructed walkways with hand rails will
be provided to cross over excavations.
7.8.2.6. Suitable barriers shall be erected around all
excavations and where necessary warning lights
shall be provided at night.
7.8.2.7. All excavations shall be backfilled as soon as
operationally practicable.
7.8.2.8. No man shall be permitted in the excavation whilst
back fill vehicle is approaching the edge or whilst
tipping fill.

7.9. Ladders And Scaffolds

7.9.1. During the course of construction work it is often required


that men work above ground level in situations where no
permanent structure yet exists or is inadequate to provide
safe working conditions.

7.9.2. In these situations ladders, scaffolds or staging must be


supplied and it is the responsibility of the supervisor of the
operation involved to ensure that all such structures are of
sound material, correctly erected and comply with Safety
Manual.

7.9.3. Ladders

7.9.3.1. Ladders either wood or metal must be of sound

construction standards and in good condition; the


supervisor shall inspect all ladders before use.

Ladder unpainted.
Site rails not split or broken.
All rungs tightly in place.
Hardware secure and movable parts operating
freely.
Extending ropes in good condition.

7.9.3.2. It is important to select the correct ladder for the


job.

Long enough to extend 3 ft. above the landing


place.
Extension ladder sections must overlap by a
minimum of 4 rungs when fully
extended.

Do not use a metal ladder or one with metal reinforcing


near exposed live electrical equipment.

7.9.3.3. When properly positioned and correctly used ladders


are as safe as any other item of equipment.

Side rails evenly supported top and bottom.


Head secured by tying or clamping. Where possible
ladder set at an angle of 75°.
Rungs shall not be used to support scaffold boards.
When a ladder is placed in an area of vehicular or
pedestrian traffic a man shall be stationed
at the foot of the ladder whilst it is in use.
One man at a time on any section of an extension
ladder.
Suspended ladders must be securely lashed top and
bottom.
No obstruction to a proper foothold at each rung.
Tools and material should not be carried in the hands
when ascending or descending.

Step ladders must be spread to their fullest extent


when in use to ensure stability.

7.9.4. Scaffolds

The two types of scaffold in use are Tube, Coupling and


system scaffold of various manufactures, in certain
circumstance the two may be successfully combined to extend
their range of application.

7.9.5. Erection

7.9.5.1. Scaffolds shall only be erected, altered and dismantled


by trained personnel under the control of an
experienced supervisor.

to the nature of the platform and its probable
load.
Ledger braces fixed at alternate standards.
Longitudinal or « dog leg » bracing to full height.
Scaffold tied in or brace rakers fitted where tying in is
impracticable.
Working platform fully decked, with planks correctly
positioned and secured in place.
Guard rail and toe boards correctly positioned.
Ladders of adequate length supplied and secured in place.

7.9.5.2. System scaffolding will vary in design, method of erection


and type of bracing they must be assembled in accordance
with the manufacturer’s instructions.

The height of an independent tower scaffold shall not


exceed 3.5 times the minimum base
dimension. For mobile Towers shall not
exceed 3 times the minimum base width.
Tower scaffolds over 32 ft. in height must be tied in or
otherwise suitably anchored.
Mobile tower scaffolds should only be used on firm and
level ground.
Men and materials must be removed from the platform
before scaffold is moved.
Wheel brakes must be fitted and must be applied when the
tower is in use.

7.9.6. Inspection

7.9.6.1. Before commencing assembly of any scaffold, the


supervisor of the erection crew will inspect all components
and reject any item found to be faulty.

06 The supervisor of the erection crew will
inspect all scaffolds weekly and after
periods of inclement weather.
07 Competent person will be assigned to
supervise scaffolds over the site as he will
issue notices to use or the “Do Not Use”.

7.10. Welding And Cutting

7.10.1. Generally arc welding and gas cutting are safe operations and
not injurious to health providing necessary precautions are
taken and basic safety rules observed.

7.10.2. The supervisor of any operation which involves gas cutting or


electric welding must ensure that the equipment to be used
complies with the Safety Manual.

7.10.3. Field supervisor will inform the safety officer that a hot work will
be carried in a particular area. The safety officer will inspect
area of work, determine the necessary precautions to be taken,
examine the skill of persons who will carry the hot work. After
that the safety will issue the “Hot Work Permit”. Also, the safety
officer will report to the safety engineer the occurrence of hot
work.

7.10.4. Oxy-Acetylene

7.10.4.1. Storage and handling :

Cylinders will be stored in properly constructed racks


which shall be covered as protection against direct sun
rays.

Oxygen and acetylene cylinders shall be


stored separately.
Full and empty cylinders shall be stored in separate
compounds which must be clearly signed.
All cylinders shall be stored secured in the upright position
with the valve cap in place.
Empty cylinders must be returned immediately to the store.
Storage compounds should be constructed as far as
practical away from buildings and other
storage areas, particularly from flammable
materials.
No smoking rule shall be enforced in the vicinity of storage
compounds.
The handling of cylinder during loading and unloading shall
be done with care; they must not be dropped,
thrown or dragged.
When being transported on vehicles the valve cap should
be in place and the cylinder secured to avoid
violent movement.
Empty cylinders are dangerous and must be treated with
the same care as full ones.

7.10.5. Inspection And Operation

7.10.5.1. Only approved equipment shall be used and should be


inspected immediately before work commences and any
faulty items replaced.

Cylinders, valves, regulators must be kept clean and free


from grease and oil.
Cylinders in use should be in a trolley and secured in an
upright position.
Connections and valves should be examined weekly for
leakage using soapy water
Hose connections must be made with clamps or by
crimping never use wire.
Ignition is to be by use of a flint gun, not a naked flame.
Do not allow the torch flame to come in contact with the
cylinders or hang lighted torch on the
regulator.

All oxy-acetylene cutting equipment will be fitted with flash


back arrestors.

7.10.5.2. Acetylene under pressure can become unstable and


explode; the pressure at the regulator must never exceed
15 PSI.

7.10.6. Emergency Action

7.10.6.1. Occasional small explosions and flame outs can occur


during use and usually are quite harmless and easily
corrected. A flashback can, however, be dangerous and in
this event the operator must take immediate corrective
action.

Close both torch valves, oxygen first.


Close both cylinder valves.
Extinguish hoses if alight.
Remove regulators and check the damage.
Inform supervisor and do not resume work until he has
inspected the equipment.

7.10.6.2. If on inspection, the acetylene cylinder appears to be over


heated or to have developed a « Hot spot » the supervisor
must:

Clear all unnecessary personnel from the area.


Remove the cylinder to an open space and open the
valve.
Apply copious quantities of water or if possible immerse
the cylinder completely in water.
Inform the site Safety Engineer.

7.10.7. Electric Arc Welding Only approved equipment shall be


used and should be inspected before work commences
and any faulty item replaced.

All connections must be secure and well contacted.


Cable insulation should be free from cuts or abrasions.

Where possible cables to be supported overhead or


covered to protect them from damage.
Any joints will be made with cable couplings and fully
insulated.

7.10.8. General Requirements

7.10.8.1. Fire can be serious hazard whenever welding or gas


cutting operations are in progress.

Remove all flammable material from the work area or


cover with a fire resistant blanket. The
blanket type should meet local regulations
and standards.
Provide metal bins for spent electrode stubs and other
scrap.
Never weld or cut on old empty drums, they may have
contained volatile liquids and remaining
vapors can be highly explosive.
A general purpose dry powder fire extinguisher must
always be available whilst work is in progress
and all mobile welding generators should be
equipped with a similar type of extinguisher.

7.10.9. Personal Protective Equipment

7.10.9.1. The appropriate personal protective equipment to be used


whilst involved in cutting and welding operations will
depend upon the particular activity being performed.

Tinted goggles are to be worn whilst cutting and goggles


with clear lenses when clipping or grinding.
Welders’ helmets with approved dark lens must be
used during electric arc welding.
Leather gloves, aprons and jackets or protective
sleeves should be used as protection
from sparks, molten metal and hot slag.
Safety boots with steel toe cap should be worn.
When welding at height, sparks will be prevented from
falling by using wood obstruction or fire
blankets where necessary.

7.10.9.2. It is occasionally necessary to perform cutting and


welding at some height above ground and in situation
where the provision of full scaffolding is not practical. In
such circumstances, safety harnesses must be worn by
all personnel involved in the operation.
7.10.10. Confined Spaces

The main risks for persons entering confined spaces are


entrapment, asphyxiation or drowning.
Training to entry, as well rescuing, of confined space will be
carried prior to executing any activity.
A job hazard analysis will be carried prior to commencement.

7.10.10.4. Proper access, lighting, ventilation &communication will be provided.

7.10.10.5. An Entry Permit to confined space will be issued after


providing the control measures.
All welding and gas cutting produces dust and fumes many of
which are irritants or toxic. In normal operations there is
ample ventilation to ensure that the welder does not
inhale enough of these substances to cause a problem,
when working inside confined spaces however this
does not apply and forced ventilation may be required.
No operation of this nature should be commenced
without first establishing safe working practices in
consultation with the site Safety Engineer, and
reference to the Construction Safety Manual.
7.11. Cranes And Lifting Gear

7.11.1. Crane accidents usually occur when the crane is being used
incorrectly or beyond its designed capacity. Supervisors must
familiarize themselves with the requirements of the Safety
Manual and the capabilities of cranes and lifting gear to be used
in operations under their control.

7.11.2. Mechanical

7.11.2.1. Cranes and lifting gear must be in good mechanical


condition free from defects and assembled in accordance
with the manufacturer’s instructions.

No crane may be operated until first passed as serviceable


by a crane inspector and a sticker issued.

All cranes are to be equipped with safe working load


indicators at such a position as to be easily
observed by the operator.
A loading chart shall be fixed inside the cab.
All controls are to be clearly marked.
Hooks must be fitted with a serviceable safety catch or
with wire.

7.11.3. Operator

7.11.3.1. The operator must possess and have in his possession a


Crane Operators License and have satisfactory passed the
operators test. He must be physically fit and mentally
alerted, an unfit operator must be removed from the crane.

He will only operate a crane of the type and rating for


which he is certified.
He will know and comply with the standard lifting hand
signals.
He will check the stability of the ground where he is to
operate and if necessary require timber
packing.
He will under no circumstances leave his crane unattended
whilst supporting a load.
Check the condition of all ropes.
Check the load indicator.
Check that all controls operate smoothly and safely.
Cab clear and windscreens clean.

7.11.4. Rigger
7.11.4.1. The position of rigger is very important as he is the only
person authorized to give signals to the crane operator.

7.11.4.2. Rigging and slinging of loads must only be carried out by a


trained and competent rigger.

He must be thoroughly conversant with the capabilities of


the crane in use.

He must be familiar with the standard hand signals and

He will normally work with the same operator.
He must be alert at all times not only to the operation he is
performing but to other operations nearby.

7.11.5. Lifting Gear

7.11.5.1. The lifting gear most commonly used is steel wire ropes or
eye and eye fiber belt slings and various types of shackle.

7.11.5.2. The rigger must examine all items of gear before


commencing work.

Any item of lifting gear showing signs of damage or


excessive wear shall not be down rated but
withdrawn from service.
Slings must be stored away from the danger of accidental
damage.
Bulldog clips, where used, must be correctly fitted with the
saddle on the live part of the rope.
Shackles will be fitted with the correct pin; rebar or mild
steel bolts are not to be used.

7.11.6. Operation

7.11.6.1. It is the responsibility of the supervisor of the lifting


operation to indicate the weight of the load to the operator
and to ensure that the SWL is not exceeded at any time.

Cranes should not be operated within a safe distance


from any power line, however, should this
become operationally essential and for
dead power cables, the operation shall be
preplanned
Loads are to be lifted smoothly and never dragged

When slewing, a minimum of 2 ft. shall be maintained

Tag lines shall be attached to all loads and manned
whilst lift is being performed.

7.11.6.2. Multiple lifting operations impose their own problems


and restrictions, no operation of this nature should be
commenced without first establishing safety.
7.12. Small Tools

7.12.1. The classification “small tools” is applied to all hand and minor
power tools.

7.12.2. GSI shall provide an adequate supply of tools


of the best quality, manufactured of sound material free from
patent defects. All such tools shall comply with the requirements
of the Safety Manual.

7.12.3. Storage And Maintenance

7.12.3.1. A clean dry store shall be provided and a record issue


and receipt maintained.

7.12.3.2. Tools in store shall be regularly inspected and cleaned


in the case of electrically powered tools this inspection
shall be performed by a competent electrician.

7.12.3.3. Damaged and excessively worn tools shall be


withdrawn from use for repair or replacement.

with a grounded plug and any splices made off properly not
merely taped. Before use, every tool must be checked to
ensure it conforms with the supply voltage and switches or
controls function correctly.

7.12.3.10. Grinders

Grinders must be fitted with a protective guard. The maximum running


speed must be clearly displayed and only grinding wheels
of a suitable periphery speed shall be used.

Only competent persons shall be allowed to install


grinding wheels which shall be examined
for defects before being mounted.
7.12.3.11. Circular Saws

Circular saws shall be fitted with spring loaded guard.

The air compressor used to supply these tools should


always be attended and hoses of the
correct size supplied.

Any joint in the hose should be made with a coupler


and an inline filter should be fitted in each
line.

7.12.3.13. Cartridge Powered

The use of cartridge operated tools is restricted to


those personnel who have successfully
completed the applicable training.

The site Safety Engineer is to be informed of the


intention to use this equipment and will
advise on Safety Standards.
7.13. Electrical

7.13.1. The provision of safe temporary electrical supply to site is the


responsibility of the Electrical Supervisor who must be fully
aware of the regulation contained in the Safety Manual.

7.13.2. The requirement for varying voltages, phases, current and the
type of electrical equipment needed must all be preplanned.
7.13.2.1. A Competent electrician shall be appointed as
responsible for the maintenance, alterations,
extensions and testing of the installation.
7.13.2.2. The installation shall be inspected daily with special
emphasis on cables and cords which are prone to
mechanical damage.
7.13.2.3. Where joints in cables are required they shall be
properly made.

and hazard operation analysis.

7.14.3.1. The correct preparations can greatly assist in


assuring a safe and efficient test procedure.

always been regarded as "dangerous" while offices have


been thought of as being safe. However, there is a great
potential for accidents in offices as there are on construction
sites, although the risks may be less obvious. Each year
there are many serious injuries sustained by office staff.
Slips, trips and falls are the most common cause of injury in
offices, those occurring on the same level being almost as
numerous as those on stairs. Next comes the handling and
lifting of goods, materials and equipment, followed by
stepping on and striking against things, falling objects,
machinery, transport, the use of hand tools, fire and
electricity.

7.18.2. This procedure applies to offices at each Contractor Project


site.

1 The office is like any other work environment in that it may


present potential health and safety hazards. Most of these,
however, may be minimized or eliminated by designing jobs
and workplaces properly, and by taking into account
differences among tasks and individuals.

7.18.4. Basic Requirements for Offices.

7.18.4.1. Office must be kept clean,

7.18.4.2. There must be adequate floor space for every person.

7.18.4.3. Temperature of 60.8F must be achieved after the first


hour.

7.18.4.4. There must be an adequate supply of fresh or purified

All Contractors and Sub-Contractor employees in the buildings must be


trained in fire evacuation procedure, every six months hold a fire
drill.

Test the fire alarm every week.

Keep records of weekly fire alarm test and six monthly fire drills.

Keep fire escape routes and doors clear.

Do not jam fire doors open.

Gets fire fighting appliances serviced regularly.

Carry out spot checks on fire fighting appliances to ensure they work
properly.

Never leave lighted cigarettes, always extinguish them.

7.18.11. Lighting.

Never mix toilet cleaning agents - they can react with each other and
produce lethal gases.

7.19. Emergency Control and Evacuation

7.19.1. Purpose of the emergency control and evacuation procedure. The


purpose of this procedure is to ensure a co-ordinate response to any

An emergency situation can occur at any time of the day, so it is


essential that. ALL personnel are familiar with and fully
understand this procedure.

7.19.2. Scope Typical situations for which a partial or full evacuation could be
considered are:
7.19.2.1. MAJOR EXPLOSION is imminent or possible.
7.19.2.2. HAZARDOUS MATERIALS incident of major consequence
has occurred.
7.19.2.3. A BOMB THREAT or CIVIL DISTURBANCE occurs.
7.19.2.4. MAJOR FIRE which affects internal or external resources.
7.19.2.5. EARTHQUAKE or MAJOR BUILDING COLLAPSE. The extent
to which personnel will be evacuated is dependent on the scale
and type of emergency situation, and where on site it occurred.

7.19.3. Responsibilities In an emergency situation, ----- representative will co-


ordinate all aspects including evacuation, use of other Emergency
Services as may be required, and return to work, when emergency is
over. ------ responsibility is to respond according to this procedure and
follow the instructions of the safety engineer. The safety engineer will:

7.19.3.1. Take control of the situation until required Emergency


Services arrive.
7.19.3.2. Co-ordinate the evacuation of personnel.
7.19.3.3. Ensure that any required Emergency Services, such as Fire
Brigade, SES, and Ambulance, etc. have been called.
7.19.3.4. Ensure that Fire Protection Systems are operating satisfactorily.
7.19.3.5. Ensure that Electricity, Process Pipe work and other services to
the effected area are isolated.
7.19.3.6. Alert adjacent areas of any possible need to evacuate.
7.19.3.7. Co-ordinate the commencement of any salvages operation and
the orderly return to work of personnel. -------- Site Manager shall
also:
7.19.3.8. Ensure that all personnel are accounted for.
7.19.3.9. When appropriate, assist in the resumption of work for - personnel
and prepare any reports as may be necessary.

Safety Officer/Supervisors will:


7.19.3.10. Provide checklists of all personnel that are on site in the area for
which they are responsible.
7.19.3.11. Assist in the evacuation of personnel from their areas.
7.19.3.12. At the Evacuation Assembly Point, conduct a” roll call” to account
for all personnel under their control.
7.19.3.13. Assist in a search for any personnel as may be required. All
Personnel will:
7.19.3.14. Evacuate the effected area in an orderly manner and proceed

directly to the appropriate Emergency Evacuation Assembly Point.


7.19.3.15. Return to work in an orderly manner when directed to do so.

7.19.4. Declaration of Emergency


When an emergency situation occurs and the decision to initiate a full or
partial evacuation has been made, it will be communicated across the site
or to the particular area by one or all of the following means:

7.19.4.1. Sounding of an alarm if one is installed.


7.19.4.2. Verbally face to face.
7.19.4.3. Telephone.
7.19.4.4. Site radio network.

The safety engineer will have the responsibility for co-coordinating


and managing the emergency situation. He/she will arrange, if
considered necessary, Assistance from external sources such as
Fire Brigade, Police, SES and Ambulance, etc.

7.19.5. Evacuation Procedure


When advised of the need to evacuate, either the whole site or a particular
area

7.19.5.1. Remain calm.


7.19.5.2. Where appropriate, leave plant/machines in a safe condition and
vehicles or equipment clear of access ways.

7.19.5.3. Walk to the nearest exit and go DIRECTLY to your Evacuation


Assembly Point (EAP). DO NOT go to change rooms, lockers and
toilets, etc.
7.19.5.4. Assemble at the Evacuation Assembly Point and have your name
checked off.
7.19.5.5. Follow directions from OH&S OFFICER or appropriate
Management Representative.
7.19.5.6. DO NOT return to work or leave the Evacuation Assembly Point
unless instructed to do so.
7.19.5.7. If working in another area, report to the Evacuation Assembly
Point for that area.

7.19.6. Evacuation Assembly Points (EAP) for Personnel.


The Evacuation Assembly Points for personnel are defined by
Construction Manager and Safety Manager. Signs are located around the
site that indicates the procedure and Evacuation Assembly Points.

8. SUB-CONTACTORS
Sub-Contractors and Sub-Contractor’s personnel must observe the above
precautions and comply with the above procedures. It is the responsibility of the

appropriate Manager to ensure that co-ordination responsibilities in this respect are


clearly established, understood and carried out by the contractor and his personnel .
Moreover, every subcontractor will have to assign a safety representative to co-ordinate
and take actions. Precautions and control measures resulting from specific activities of
the subcontractor will be included contractually in his responsibilities.

9. TRAINING
It is the policy of ------------------------ to actively provide safety training for all
employees. ------------------------ recognizes that an effective training program will
requires cooperation and consultation between management and employees.
Safety training will be conducted internally and externally, using formal and informal
formats as considered necessary. A record of safety training undertaken by
------------------------ employees will be kept in the safety section of the filing system.
The Personnel Department will maintain these records.

employment on site. These safety orientations designed to develop safety
awareness and are introduced for Occupational Health and Safety Program.

10. JOB HAZARD ANALYSIS

10.1. The construction of a project is manifested through the execution of


sequential activities. Each activity can be considered hazardous respect to
the harm that can cause to a person, asset and the environment. Activities
will be selected and filtered from a scheduling program, and each of these
activities will be examined through a Job Hazard Analysis and the steps will
be as follows:
10.1.1. Select all hazardous activities.
10.1.2. Highlight the hazards.
10.1.3. Identify the risks, and
10.1.4. Who might be at risk
10.1.5. Perform risk assessment and Risk Rating
10.1.6. Allocate the necessary control measures
10.1.7. Re-evaluate the risk for sufficient low level.

10.2. The execution of any activity requires elaboration through a method


statement in which certain details will be included and it will clarify the
process, location, manpower, equipment and material. As a result a clear
view of the hazards can be concluded enabling the JHA process to start
adequately.

11. RECORDS

All records relevant to above procedure shall be kept by the Safety Engineer in the
site office for the duration of the project and transferred to the Area Head Office, at
the achievement of the Project, to be filed, archived and kept for a period of Ten
years.
