1) KTSB personal i.e either Ops. Or EMP are the only valid requestor. No contractor are allowed to do the request
2) Period of Defeating must not exceed more than 2 months (including Extension). Any defeat required more than 2 months must be approved by Operations Manager only
3) Any DSCD require more than 3 months must cover through an MOC
4) Shift supervisor only can approved DSCD that owill not tripped the system i.e cover with redundant or standby unit
5) Ensure DSCD log is timely updated and close Rev_02 (02nd Aug 2016)
