I. PURPOSE
This procedure will serve as a guidance in conducting inspection and maintenance of first aid
box/cabinet inside International Pharmaceuticals, Inc.

II. SCOPE
This procedure is a company-wide undertaking that should be complied by all members of the
Emergency Response Group and Maintenance personnel of International Pharmaceuticals Inc.

III. RESPONSIBILITIES
Roles and action steps are described within the process section of this SOP. Activities may be
delegated to an appropriately qualified individual; however, accountability is not delegated.

A. President and CEO, EVP and COO, Division Heads and Department Heads are responsible for:
1. Ensuring compliance to this procedure.

2. Ensuring that all relevant personnel will undergo training on this SOP.

B. The President and CEO, EVP and COO are responsible for:
1. Providing necessary support and resources for the timely implementation of this
procedure.

C. The Environmental, Health and Safety Department is responsible for:

1. Conducting monthly inspection and maintenance to all first aid cabinet installed in their
areas of responsibility.

1. Conducting regular audit to ensure this procedure has been followed.

2. Issuing non-compliance to Area Emergency Responders once observed not following this
procedure.

H. The Environmental, Health and Safety Head is responsible for:


1. Ensuring that this procedure has been understood by all personnel concerned.

2. Ensuring that EHS Officers monitor compliance to this procedure.

I. The Human Resource Department Head is responsible for:


1. Providing disciplinary actions to members of the ERG who are non-compliant to this
procedure.

IV. GENERAL
A. This procedure has been written in compliance to OSHA 1910.151, it states that employees be
given a safe and healthy workplace that is reasonably free of occupational hazards. However,
it is unrealistic to expect accidents not to happen. Therefore, employers are required to
provide medical and first aid personnel and supplies that corresponds with hazards of the
workplace.
1. It is stated in 1910.151.a that the employer shall ensure the ready availability of medical
personnel for advice and consultation on matters of plant health.

2. In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which
is used for the treatment of all injured employees, a person or persons shall be adequately
trained to render first aid. Adequate first aid supplies shall be readily available.

B. In addition, as stated in OSH Rule Annex A 1.d that there shall be adequate physical provisions
for the safety of laboratory personnel considering exposure to chemicals, inflammable
reagents, fires, and similar substances. Safety provisions shall include emergency exit and

egress, emergency shower and eyewashes, fire extinguishers, first aid kits, fume hoods and
appropriate personal protective equipment.

C. Furthermore, ANSI/ISEA Z308.1-2021 establishes minimum performance requirements for first


aid kit and supplies. First Aid Kit are classified based on the assortment and quantity of first
aid supplies intended to deal with most types of illness that may be encountered in the
workplace. As each work environment is unique, it is expected that the contents of each kit
will be supplemented as needed based upon on the recommendations of a person competent
in first aid. Classification of first aid kit are as follows:
1. Class A is designed for common workplace injuries such as major and minor wounds,
minor burns, and eye injuries. This first aid kit should include bloodborne pathogen kits.

2. Class B is designed for high-risk work zones such as contaminants, utilities, industrial and
manufacturing. This is recommended for any work zone where personal protective
equipment is needed.

A. Each class of first aid kit is recommended with types of containers and is classified by
portability, ability to be mounted, resistance to water, corrosion, and impact
resistance. This are the four types of containers based on the classes of first aid kit:

Types of first aid kit Usage


Type 1 Intended for use in stationary, indoor applications where kit
contents have minimal potential for damage due to
environmental factors and rough handling. These kits are not
intended to be portable and should have a means for mounting
in a fixed position. Some applications for Type I first aid kits are
general indoor use, office use, or use in a manufacturing
facility. First aid cabinets would generally fall into this type.
Type 2 Intended for us in portable indoor applications where the
potential damage due to environmental factors and rough
handling is minimal. These kits should be equipped with a
carrying handle. Some applications for Type II first aid kits are
general indoor use, use in an office or manufacturing
environment.
Type 3 Intended for portable use in mobile indoor and/or outdoor
settings where the potential for damage due environmental
factors is not probable. Kits should have the means to be
mounted and have a water-resistant seal. Typical applications
include general indoor use and sheltered outdoor use

industry, construction industry and the armed forces.

E. The ANSI/ISEA Z308.2-2021 states that first aid kit should be inspected at least monthly or
following a first aid incident when product is used. Cabinet and other tools should be
disinfected regularly. Employees or building occupant should know their roles and
responsibilities about first aid, have access, and establish inspection schedule.

F. Area Emergency Responders should be acquainted with the first aid cabinet installed in their
areas of responsibility and shall be trained on how to conduct inspection and maintenance. As
indicated in ANSI/ISEA, periodic inspection of first aid cabinet shall include a check of at least
the following items:
1. Location in designated place.
2. Obstruction of first aid kit
3. Examination for obvious physical damage of the box/container
4. Volume of each item
5. Expiration date
6. New and Old items/products
7. Opened items
8. Damaged items

G. This procedure shall be complied by the designated Area Emergency Responders, Area
Coordinators and Area Managers. In the event that this procedure has not been complied by
the designated personnel, a disciplinary action will be given as deemed appropriate by the
management.

V. DEFINITIONS
TERM DEFINITION
ANSI American National Standards Institute is the main organization
supporting development of technology standards in the United States.
ANSI works with industry groups and it is the US member of the
international Organization of Standardization (ISO) and the
international Electrotechnical Commission (IEC).
First Aid Kit A first aid kit is a box, bag or pack that holds supplies used to treat
minor injuries including cuts, scrapes, burns, bruises, and sprains.

equipment and technologies – equipment and systems that enable


people to work in hazardous environments.
OSHA Occupational Safety and Health Administration is a large regulatory
agency of the United States Department of Labor that originally had
federal visitorial powers to inspect and examine workplaces.
OSHS Occupational Safety and Health Standards was formulated in 1978 in
compliance with the constitutional mandate to safeguard the worker’s
social and economic well-being as well as his physical safety and health

VI. PROCEDURE
A. Designated Area Emergency Responders
1. Conduct inspection to installed first aid cabinet in their areas of responsibility using form
no. EHS4720 “First Aid Cabinet Inspection Checklist”, Attachment B.

2. Fills out correct information including the firs aid cabinet code and location using the
form EHS4720.

3. Confirms the first aid cabinet is unobstructed and in its designated location. In the event
that it is obstructed and not in its designated location, report to Area Coordinator to
provide immediate rectification.

4. Examine the first aid cabinet for obvious physical damage. In the event that the
emergency first aid cabinet is damaged, informs Area Coordinator to request job order
from GSD.
5. Check the number of items in the checklist. In the event that the first aid kit item does
not match with the checklist, informs Area Coordinator to investigate the lacking items
from the issued log book.

6. In the event when the first aid kit items were used during treatment of injury, log items
used in the log book. Informs Area Coordinator to request for replacement in the EHS
Department using Material Request Slip in SAP with attached complete filled out
Incident Report and EHS4720 “First Aid Cabinet Inspection and Maintenance Log”,
Attachment C.

7. Check the expiration date in every item in the first aid box. If the item is expired, informs
Area Coordinator to request for replacement in the EHS Department using Material
Request Slip in SAP.

8. Check sealed and opened items. In the event that the items sealed is broken or the item
is open, informs Area coordinator to request for replacement in the EHS Department
using Material Request Slip in SAP.

9. Segregate the old items from the new items, arranged them accordingly based on
expiration date. “First In, First Out” rule shall be implemented.

10. Make sure to maintain monthly log for inspection, place date and signature over printed
name and places check mark to EHS4720-F2, and after all first aid kit in the area has
been checked, submits signed e-file to Area Coordinator for review and signature.

1. Designated Area Coordinator


a. Receives monthly reports from the Area Emergency Responders and provide
necessary actions such as immediate rectification.

b. Process Material Request Slip to Environmental, Health and Safety Department


requesting to restocked used items and attached it with EHS4720-F2 “First Aid
Cabinet Inspection and Maintenance Log”, Attachment C, and complete filled out
Incident Report.

c. Process Material Request Slip to EHS Department requesting to provide EHS4720


“First Aid Cabinet Inspection and Maintenance Tag”, Attachment D.

d. Requests for replacement of damaged first aid cabinet and submits Material
Request Slip to EHS Department.

e. Process Job Order to General Service Department.

f. Reviews and sign the e-file and send it to ehsdepartment@ipi.ph and carbon copy
in the email the Area Manager, EHS Department Head, and designated EHS Officers
in charge of first aid cabinet.

g. Receives report from EHS Officer regarding any deficiency in the submitted form
EHS4720.

h. Ensures to comply within 5 days regarding the deficiency in the submitted


inspection report.

i. Submits report to EHS Department Head carbon copy Area Manager regarding
non-compliance of Area Emergency Responders and takes over the responsibility in
checking extinguishers to ensure its readiness in times of emergency in their area
of responsibility and submits inspection report within the given 5 days grace
period.

B. EHS Officers
a. Receive job orders relating to request for replacement of damaged first aid cabinet and
used provision of EHS4720-F1 and EHS4720-F2.

b. Receive EHS4720-F1 through email and confirms receipt conducts audit to validate
reports from the Area Emergency Responders.

c. Send e-file to EHS Department Head regarding the verified reports from Area
Coordinators.

d. Inform the Area Coordinator regarding any deficiencies in the submitted report to
comply within 5 days.

e. Issue non-compliance to Area Coordinator/s and Area Emergency Responders and


carbon copy Area Manager/s and Division Head/s in the event of failure to comply with
inspection and maintenance checking of first aid cabinet on the given period of
compliance and informs the Area Coordinator to take over the responsibility in checking
first aid cabinet.

C. General Services Department


a. Receives job order and process request.

D. EHS Department Head


a. Receives reports, places e-signature and reverts back to the EHS Officer.

b. Communicates with Department Heads referred to as Area Managers regarding the


performance of Area Coordinators and Area Emergency Responders.

c. Endorses report to HRD Head for further disciplinary actions.

E. Human Resource Department Head


a. Receives report from the EHS Department Head and provides disciplinary action to
erring Area Coordinators and Area Emergency Responders.
