Direction: Make a contract of commitment on fitness by providing your fitness goals and the
target timeline to achieve it. Please see sample of contract of commitment on Fitness and also the
rubrics to assess your output.

Fitness Goals 2021:
 Sweat More. Exercise more.
 Burn Fats.
 Balanced Healthy Eating Habits.
 Stretch & Strengthen.
 10,000 Steps a day.
 Rest and Recover.
 Positive & Happy Mindset.
 More Water. Stay Hydrated at all times.
 Do something active that calms me twice a week.
 Have at least 6-8 hours of sleep.
 Be Consistent, Commit & Change

I, _Shanley P. Mangomayao_, commit myself to commence a healthy and active


lifestyle. I will commit to my fitness goals starting on _January 24, 2021_, up to
_February 14, 2021_, for a minimum of _120 minutes a day, and I will remain
motivated committed and optimistic. I personally believe my goals for health and fitness
goals were indeed realistic and which I can fulfill them.
I take into account that I might encounter various activities in life which could
interfere with my fitness goals but, I promise to do my best to stick to my commitments
that aims to help me in having a healthy and happy life in the long run and also
encourage my family, friends with their Fitness Goals.
Prior to the actual conduct of my goals and activities, I take full accountability for
acknowledging my wellbeing. There is, indeed no excuse for me to miss and ignore my
fullest commitment in the activities and goals to my lifestyle changes.
