2. Explain how data collection & analysis is employed to assess the health status of
each family.

3. Present diagnoses for different cases based on various data presented.


Activities Directions for Activities:
1. Two parts of audio recordings on the following topics will be uploaded:
1.1 Introduction to Family Assessment until Data Collection
1.2 Data analysis up to Family Assessment Tools
2. The uploaded audio recordings come with a pdf copy of the powerpoint presentation
and a copy of the Community Diagnosis Form that shall serve as guide in
presenting the topics .
3. Do the following tasks with instructions:

Instructions:
HPS – 40 points 1. Listen and understand the audio recordings on the following topics which will be uploaded
in two parts:
1.1 Introduction to Family Assessment until Data Collection
1.2 Data analysis up to Family Assessment Tools

2. Have a copy of the Community Diagnosis Form at hand to do the following


instructions:
2.1 Use a copy of the Community Diagnosis Form in accomplishing this activity.
2.2 On the form, highlight each of the following indices as to which of these indices
require plain observation, Physical Examination or Interview to collect data. Use
the following color codes to present your answer:

Green- If the indice/indices may be collected by just observation


Yellow-If Physical Examination is required to collect data.
Orange- If Both Observation & Physical Examination should be used to collect data.
Blue- If plain interview will collect the necessary data.
Purple- If observation and Interview should be used to collect the data.
Red- If Observation, Physical Examination & Interview are necessary to gather data.

2.3 Submit the accomplished form with the set deadline.

3. Read Module 1 and answer assessments 1 & 2.

3.1. The class will be divided into 6 groups with 7 to 8 members for each group.
3.2 Each group will be assigned to study the following indices/data to be collected
as contained in the Community Diagnosis Form.
Group 1- I. Analysis of the Population (IA (Household Composition)
to IE (Environmental indices)
. Group 2- II. Analysis of the Health Sector ( II.A Child birth to II.C
Maternal/Prenatal Supervision)
Group 3- II. Analysis of the Health Sector (II. D Family Planning Practices to
II.E General Health Practices)
Group 4-III. Analysis of the Health Status ( III.A. Morbidity Record & III.B
Mortality
Cases)
Group 5-IV. Knowledge of Primary Health Care Concepts
Group 6- V. Perceived Community Health Problems/Solutions
& VI. Access to Health Service Program

3.3 The questions for each of those indices should be divided among the members
of the group.Each member should study the assigned questions to him/her.
3.4 Each member should conduct a brief interview with the questions on hand. The
student-interviewer may select any family member who currently stays with him
or her in the same house.
3.5 Confidentiality form may be accomplished and provided if needed.The
students-interviewer should ask for permission to take videos/audios prior to
making any records.
3.6 Each videorecording should be limited to 2 to 3 minutes per member regardless
of the length of the interview. Only a 2 to 3 minute portion of the interview
should be taken and compiled with all the the rest of the group’s
videorecordings.
3.7 The videorecording for each group should be limited to 15 minutes only to
present the entire part of the assigned data to collect. The videorecording
should include the fff. Information with the video:
 Grade & Section
 Group Number and the Indice/Indices assigned to the group
 full names of the members ( alphabetically-arranged)
 Assigned questions opposite the names
 Date submitted
3.8 Submit the videorecording in MP4 format inside a file folder named after your
Group number and the indices assigned.
