MECHANICS ON DISTRIBUTION & RETRIEVAL OF

SELF LEARNING MODULES
(to be done by parents/guardian)

DISTRIBUTION:
1. Parents should observe health protocols:
* Wearing of face mask and face shields.
* Wash hands with soap & water and sanitize it with alcohol.
* Proceed to the thermal scanner in-charge for temperature check.
* Fill-out the contact tracing log book.
* Maintain two-meter physical distancing while you are in school premises.
2. Fall in line outside the classroom in the designated waiting area.
3. Take turn in the distribution of modules. (Only 1 person is allowed to enter.)
4. Upon entering the classroom, look for the name of your child posted on the
chairs then check the complete set of modules.
5. Affix your signature in the distribution tracking sheets.
6. Then, follow the indicated arrow for exit.

RETRIEVAL:
1. Parents should observe health protocols:

4. Upon entering the classroom, present the complete set of modules, answer
sheets, notebooks/output to the teacher.
5. Affix your signature in the retrieval tracking sheets.
6. Check the seat plan or designated chairs of your child and place the complete
set of modules, answer sheets, and notebooks/output.
7. Then, follow the indicated arrow for exit.
