TABLE OF CONTENTS:
Family Serrano lives in Makinabang , Baliuag, Bulacan. The head of the family is Mr.Serrano a
truck driver and responsible for the health and welfare of the family, his wife Mrs.Serrano is a
house wife and sometimes do extra jobs to help for the financial support of the family. They are
blessed of five children with an initial of G. Serrano as the eldest, M.D Serrano, Y. Serrano, M.
Serrano, and the youngest among the siblings is K. Serrano who are our main subject, kendra is a
Female and currently 4 yrs old she became prone to sickness at the age of 3 and tends to always
have a repeating sickness such as fever, cough and cold. Both Mr & Mrs Serrano are not always
at home and unable to take care of their children, since most of the time Mrs. Serrano isn’t always
at home the eldest daughter is the one who looks out for her siblings. But she wasn’t able to look
out for her siblings at all times because she is responsible or in-charge also of doing household
chores and also she have modules to answer since she is still in the elementary level. While Mr.
Serrano , works as a truck driver and wasn’t able to go home because his route is around Region
3 and NCR, since the salary of the truck driver is in minimum wage, he wasn’t able to provide the
needs of the family also because their utmost priority is to pay their debts and other expenses.
Because of this situation the children faces problem concerning their health and since the parents
are always busy and have hectic schedule the children unable to visit the nearest health center.

V. DOCUMENTATION
The interview was made through personal at the house of the family, before the start of the
interview we ensure that we observe safety protocols such as hand washing, proper distancing
and wearing of facemask. We make sure that we established rapport to the mother and the child
so that it will be comfortable to them to communicate with us throughout the interview and to get
the necessary information for our initial data base. Furthermore, we explained briefly the
procedure and ensured to the mother that all the information that she will be sharing to us will be
for educational purposes only and securing confidentially of information between us interviewers
and our clinical instructors for the safety of their family.
