This is to inform you that your son/daughter _________________________ of Grade 3-Jupiter,
has shown difficulties in reading that affects his/her academic performance in school. If no
intervention will be made, there will be a possibility of retaining her/him to the current grade they
are in if their performance will not be improved.
In connection with this, may we ask your permission to allow your son/daughter to attend
remedial classes from October 10, 2022-July 2023 (except Saturdays and Sundays) every
11:00-11:30 in the morning after classes.
By signing this consent, you also acknowledge your responsibility as guardian/parent to ensure
that the learner will attend the remedial classes at the said time.
