SPRINGFIELD ­ The Illinois Department of Financial and Professional Regulation 
(IDFPR) announced today that the Directors of Professional Regulation, Daniel E. 
Bluthardt; Financial Institutions, Robert Meza; and, Insurance, Michael T. McRaith, 
signed the following disciplinary orders in May 2008. Orders for the Division of Banking 
were authorized by Director Jorge Solis. 

Banks 
RESIDENTIAL MORTGAGE 

Chicago First Mortgage, Inc., Chicago – residential mortgage license 
(MB.0007013) revoked for failure to respond to supervisory requests and violations 
found in examination. 

First Community Banc Group, Naperville – residential mortgage license 
(MB.6759460) revoked for failure to provide a change of address and business activities, 
unregistered loan originators and failure to maintain records for three years. 

A & A Mortgage, Inc., Chicago – residential mortgage license (MB.6760409) 
fined $500 for failure to comply with advertising requirements. 

New Horizon Mortgage Corporation, Chicago – residential mortgage license 
(MB.0005903) revoked for failure to respond to a consumer complaint, pay an 
examination fee and other violations. 

Accurate Finance, Inc., Buffalo Grove – residential mortgage license 
(MB.0005428) fined $500 for failure to comply with advertising requirements. 

Silo Mortgage Group Corporation, Palos Hills – residential mortgage license 
(MB.6759393) revoked for failure to comply with the Department and failure to pay fine 
owed to the Department, improper surrender of license, failure to comply with net worth

LOAN ORIGINATOR 

Rosanna Cordero, Chicago ­ LO Registration (031.0010508) was fined $1,000 
for submitting completed loan files she herself did not prepare but just signed off on. 
This Order is under appeal. 

Joseph Dorf, Deerfield ­ LO Registration (031.0019016) was revoked for not 
reporting a new conviction within the required time period to the Department and due to 
the conviction being for a crime of moral turpitude.  This Order is under appeal. 

Richard Fousek, Sugar Grove ­ LO Registration (031.0014061) was fined 

$1,000 for conducting loan originating activities while not properly registered to Licensee 
at the time but still registered under his previous Licensee. 

Stephanie McLemore, Bloomingdale ­ LO Registration (031.0011598) was 
revoked and fined $38,000 for submitting a loan file with fraudulent employment 
information which led to foreclosure of the property. This Order is under appeal. 

Hiroko Smith, Chicago ­ LO Registration (031.0006885) was fined $1,000 for 
submitting a loan file with inconsistencies including inflated salary of the borrower.

Fast Cash In a Flash, Inc., 962 North McLean Boulevard, Elgin – CILA 
Licensee #2380 fined $10,850 for the following violations: payment receipt does not 
show the required elements, loan document does not accurately disclose payments, and 
security not properly disclosed. 

Pronto Finance Group, LLC D/b/a Pronto Preterm’s, 1750 Todd Farm 
Drive, Elgin – CILA Licensee #295 fined $10,250 for the following violations:  Wage 
assignment not completed correctly, incomplete disclosure of itemization amount 
financed, incorrect contract used in issuing loan, incorrect disclosure of amount financed, 
security not properly disclosed, and annual percentage rate not accurately disclosed.

related to information on the contract being wrong, incomplete, or non­disclosure of

update data base with required information on date of transaction and did not update data

Insurance 

Michael John Brdecka, Naperville – insurance producer license to sell fire and 
casualty insurance revoked effective May 8, 2008, as a result of an investigation, which 
revealed a 2006 felony conviction of Offer, Acceptance, or Solicitation to Influence 
Operations of Employee Benefit Plan.  Additionally, the Licensee failed to properly 
report the felony.

indefinitely suspended and fined $3,000 due to a federal conviction for mail fraud in 
connection with offering and selling of securities to the public.

assessed a joint civil penalty of $5,000 for conducting and reporting on an audit of a non­ 
profit corporation under Generally Accepted Auditing Standards (GAAS) without a CPA

employee, a family member, in obtaining fraudulent wages.

indefinite probation after defaulting on an Illinois educational loan and has now entered 
into a repayment agreement.
 Kimberly Derry, Abingdon – registered nurse license (041­294125) indefinitely 
suspended after testing positive for cocaine in a "for cause" drug test. 

Pamela Dietz, Waterloo – registered nurse license (041­336593) indefinitely 
suspended due to diversion of Vicodin and altering a prescription. 

Tracy Doty, Morton – registered nurse license (041­303993) indefinitely 
suspended for a minimum of five years for conviction of Wire Fraud (having defrauded 
her relative of approximately $90,000) and failure to report. 

Doris Gorter, Frankfort – registered nurse license (041­319382) reprimanded
