Sub: Regarding permission for marriage during lockdown

Respected Sir,
I, Ramchandra Nago Mali resident of House No.324, Shree Ganesh Niwas, At-Wanjarpada,
Neral, Tal-Karjat, Dist-Raigad, Maharashtra, am writing this letter to bring to your notice that
my son, Mr. Ganesh Mali’s, wedding is to take place on September 14th, 2021 at the
address Shree Samarth Krupa Marriage Hall, Gaulwadi, Kadav, Tal-Karjat Dist Raigad
and I would like to seek the permission for the same.
We are aware that during the coronavirus pandemic, the Government of India has announced
certain restrictions for wedding ceremonies. We will abide by all the rules that have been
mentioned by the government as responsible citizens. Keeping in mind the same, we have

kept our guest list to the maximum of 25 guests which include only the family members. We
are conducting just one ceremonial function on September 14th, 2021 from 3 pm to 6 pm.
We request you to please grant us the permission for the same so that we can invite our close
relatives for my son’s marriage ceremony and have the event take place without any

[1] Date of Registration of Vehicle No. MH06 AJ6798 and the date of dispatch of RC book to
the Registered Owner by your public authority.

[2] Speed Post Receipt number and date under which the RC Book was dispatched to the
registered owner.

[3] Address of the registered owner to which the RC Book was dispatched by your public
authority.

[4] In case the RC book is not delivered, but returned to your public authority, please inform
me the name, designation and mobile number of the Officer(s) and staff who should be
contacted for taking delivery of the RC book.

[5] In case the RC book is not prepared and dispatched to the registered owner till this date,
please inform me the name of officer(s) and staff responsible to prepare and dispatch the RC
book to the registered owner, but failed to do so.

[6] Certified copy of the citizen charter or any other such document stipulating the minimum
time limit within which the vehicle RC book is required to be prepared and dispatched to the
registered owner.

[7] Supply me the copy of rules, regulations, circulars, GR, notifications or any such other
document stipulating the provisions and method for obtaining the RC book, if the same is not
delivered to the registered owner even after reasonable period of 5 months from the date of
registration.

Affidavit for change in name and declaration – Summary

An affidavit is a written statement from an individual which is sworn to be true. It is an oath


that what the individual is saying is the truth. The legal document sets out the facts of the
case and tells the court that the information contained in the affidavit is 100% true and
factual. In other words, an affidavit is the written version of a sworn testimonial. An affidavit
may be given on any court petition, motion or evidence.

Mrs. Hemlata Suresh Barshi, aged 23 years, resident of Humgaon, Kadav, Tal Karjat,
Raigad got married with Mr. Ganesh Ramchandra Mali on 14th February 2021. After
marriage her name changed to Hemlata Ganesh Mali and that’s why she wants to change her
name officially and making an affidavit as a legal document. Making this affidavit to change
old name to new one from all the official documents like Domicile Certificate, Aadhaar card
and PAN Card and Election Card.

The first step in the legal procedure is affidavit submission. You need to contact a notary to
do the needful. The following steps would make the process simpler and clearer:
1. Approach your local notary and mention your request.
2. The notary will suggest the stamp paper of the required value on which the affidavit for the
Name change is made
3. Once you have the paper, you need to provide the following details:
a. Name and New name
b. Current address
c. Reason for the name change

The affidavit reflects both your old and new names. Affidavits can be used for changing the
Whole name, surname or just a few letters.

Keep a copy of the affidavit. This is a crucial document and comes in handy for any future
legal formalities.

I, Hemlata Suresh Barshi daughter of Suresh Balu Barshi and wife of Ganesh
Ramchandra Mali aged 23, residing at Wanjarpada, Neral Maharashtra, do hereby
solemnly affirm and declare as under:
1. That my maiden name is Hemlata Suresh Barshi.
2. That I got married to Shri. Ganesh Ramchandra Mali on 14th February 2021 at
Humgaon, Kadav, Karjat.
3. After marriage my name is Hemlata Ganesh Mali and all the records have the
same name.
4. I have not obtained the marriage certificate.
5. I state that Hemlata Suresh Barshi and the Hemlata Ganesh Mali is the name of
one and the same person and that is myself.

I am executing this declaration to be submitted to the concerned authorities for


the change of name. I hereby state that whatever is stated herein above are true
to the best of my knowledge.
